摘要
目的探究妊娠期糖尿病(GDM)患者血清微小RNA⁃27(miR⁃27)、微小RNA⁃34c(miR⁃34c)表达水平与胰岛素抵抗(IR)的相关性。方法选取2016年7月至2019年12月郑州大学第五附属医院收治的158例GDM孕妇为GDM组,并选取同期162例健康孕妇为对照组。比较两组孕妇一般资料及血清miR⁃27、miR⁃34c、空腹血糖(FBS)、空腹胰岛素(FINS)水平、胰岛素抵抗指数(HOMA⁃IR);分析GDM孕妇血清miR⁃27、miR⁃34c与HOMA⁃IR的关系及GDM发生的影响因素。结果GDM组孕妇体重指数、三酰甘油(TG)、低密度脂蛋白(HDL)、高密度脂蛋白(HDL)、血清miR⁃27、FBS、FINS和HOMA⁃IR水平均高于对照组,miR⁃34c水平低于对照组,差异均有统计学意义(P<0.05)。GDM患者血清miR⁃27表达水平与HOMA⁃IR水平呈正相关(P<0.05),血清miR⁃34c表达水平与HOMA⁃IR水平呈负相关(P<0.05)。体重指数、TG、miR⁃27、HOMA⁃IR是影响GDM发生的危险因素(P<0.05),miR⁃34c是影响GDM发生的保护因素(P<0.05)。结论GDM孕妇血清miR⁃27表达上调,miR⁃34c表达下调,miR⁃27、miR⁃34c与IR有关,两者可能与IR相互影响,进而影响GDM发生发展。
Objective To investigate the correlation between the expressions of microRNA⁃27(miR⁃27),microRNA⁃34c(miR⁃34c)and insulin resistance(IR)in gestational diabetes mellitus(GDM).Methods 158 GDM pregnant women admitted to the Fifth Affiliated Hospital of Zhengzhou University from July 2016 to December 2019 were selected as the GDM group,and 162 healthy pregnant women were selected as the normal group.The general data,serum miR⁃27,miR⁃34c,fasting blood glucose(FBS),fasting insulin(FINS)and insulin resistance index(HOMA⁃IR)of the two groups were compared.The relationships between serum miR⁃27,miR⁃34c and HOMA⁃IR,and the influencing factors of GDM were analyzed.Results The levels of body mass index,triglyceride(TG),low density lipoprotein(HDL),high density lipoprotein(HDL),serum miR⁃27 level,FBS,FINS and HOMA⁃IR in the GDM group were significantly higher than those in the normal group(P<0.05),and miR⁃34c was significantly lower than that in the normal group(P<0.05).The expression level of serum miR⁃27 in GDM patients was positively correlated with the level of HOMA⁃IR(P<0.05),and the expression level of serum miR⁃34c was negatively correlated with the level of HOMA⁃IR(P<0.05).Body mass index,TG,miR⁃27 and HOMA⁃IR were risk factors affecting the occur⁃rence of GDM(P<0.05),and miR⁃34c was a protective factor affecting the occurrence of GDM(P<0.05).Conclusion The expression of miR⁃27 is up⁃regulated in GDM pregnant women,and the expression of miR⁃34c is down⁃regulated.miR⁃27 and miR⁃34c are related to IR,they may interact with IR,and then affecting the occurrence and development of GDM.
