摘要
目的研究血清鸢尾素与2型糖尿病(type 2 diabetes mellitus,T2DM)患者骨骼肌质量的相关性。方法选取临沂市人民医院健康管理中心的217例T2DM患者作为观察组,62例健康受试者作为对照组,两组分别按照骨骼肌质量状况再分为骨骼肌质量正常和减少两个亚组。收集受试者人体成分和实验室参数,分析T2DM患者血清鸢尾素与骨骼肌质量的关系。结果观察组骨骼肌质量减少比例显著高于对照组(χ^(2)=4.87,P<0.05);观察组四肢骨骼肌质量和骨骼肌指数(SMI)显著低于对照组(t=2.33、2.12,P<0.05);观察组骨骼肌质量减少亚组血清鸢尾素水平显著低于正常亚组(t=6.83,P<0.05)。多元Logistic回归分析显示,血清鸢尾素水平降低是骨骼肌质量减少的危险因素(OR=0.974,95%CI=0.964~0.983,P<0.001)。结论血清鸢尾素与T2DM患者骨骼肌质量具有相关性,有可能作为T2DM患者肌少症诊断的生物学标志物。
Objective To investigate the relationship between serum irisin and skeletal muscle mass in patients with type 2 diabetes mellitus(T2DM).Methods We selected 217 patients with T2DM(case group)and 62 healthy subjects(control group)from the Health Management Center of Linyi People’s Hospital.Based on the skeletal muscle mass,each group was further divided into normal mass subgroup and low mass subgroup.Data about body composition and laboratory parameters were collected to analyze the relationship between serum irisin and skeletal muscle mass in patients with T2DM.Results The control group showed a significantly higher percentage of a loss of skeletal muscle mass(χ^(2)=4.87,P<0.05),a significantly lower appendicular lean mass(t=2.12,P<0.05),and a significantly lower skeletal muscle index(t=2.33,P<0.05)compared with the control group.Serum irisin level was significantly lower in the low mass subgroup than in the normal mass subgroup(t=6.83,P<0.05).Multivariate logistic regression analysis showed that a low serum irisin level was a risk factor for a loss of skeletal muscle mass(OR=0.974,95%CI=0.964-0.983,P<0.001).Conclusion Serum irisin is associated with skeletal muscle mass in patients with T2DM,and may be used as a biomarker for sarcopenia in patients with T2DM.
