益气活血补肾汤联合阿司匹林肠溶胶囊对冠心病不稳定型心绞痛患者NT-proBNP、MPO、hs-CRP的影响

Effect of Yiqi Huoxue Bushen Decoction Combined with Aspirin Enteric Capsules on NT-proBNP,MPO and hs-CRP in Patients with Unstable Angina Pectoris of Coronary Heart Disease

目的:观察益气活血补肾汤联合阿司匹林肠溶胶囊对冠心病不稳定型心绞痛患者N末端B型利钠肽原(NT-proBNP)、髓过氧化物酶(MPO)、超敏C反应蛋白(hs-CRP)的影响,为临床治疗提供参考。方法:选取2017年12月-2021年5月上饶市广丰区中医院收治的120例冠心病不稳定型心绞痛患者,根据随机数字法将其分为消心痛组和自拟汤剂组,每组60例。消心痛组给予硝酸异山梨酯片联合阿司匹林肠溶胶囊治疗,自拟汤剂组给予益气活血补肾汤联合阿司匹林肠溶胶囊治疗。比较两组疗效、心绞痛发作情况、硝酸甘油用量、NT-proBNP、MPO、hs-CRP及血液流变学指标。结果:自拟汤剂组总有效率为91.67%,高于消心痛组的76.67%,差异有统计学意义(P<0.05)。治疗后,两组持续时间均短于治疗前,发作频率和硝酸甘油用量均低于治疗前,且自拟汤剂组持续时间短于消心痛组,发作频率和硝酸甘油用量均低于消心痛组,差异均有统计学意义(P<0.05)。治疗后,两组NT-proBNP、MPO、hsCRP水平及全血高切黏度、全血低切黏度、血浆黏度、血细胞比容均低于治疗前,自拟汤剂组均低于消心痛组,差异均有统计学意义(P<0.05)。结论:益气活血补肾汤联合阿司匹林肠溶胶囊治疗冠心病不稳定型心绞痛效果明显,可调节NT-proBNP、MPO、hs-CRP的表达水平,改善血液流变学,减轻心绞痛症状。

Objective:To observe the effect of Yiqi Huoxue Bushen Decoction combined with Aspirin Enteric Capsules on N-terminal B-type natriuretic peptide(NT-proBNP),myeloperoxidase(MPO) and highsensitivity C reactive protein(hs-CRP) in patients with unstable angina pectoris of coronary heart disease,so as to provide reference for clinical treatment.Method:A total of 120 patients with unstable angina pectoris of coronary heart disease admitted to Guangfeng District Hospital of Traditional Chinese Medicine of Shangrao City from December 2017 to May 2021 were selected,and they were divided into Xiaoxintong group and self-prepared decoction group according to random number method,with 60 patients in each group.The Xiaoxintong group was treated with Isosorbide Dinitrate Tablets combined with Aspirin Enteric Capsules,and the self-prepared decoction group was treated with Yiqi Huoxue Bushen Decoction combined with Aspirin Enteric Capsules.The efficacy,angina pectoris attack,Nitroglycerin dosage,NT-proBNP,MPO,hs-CRP and hemorheology indexes were compared between two group.Result:The total effective rate of the self-prepared decoction group was 91.67%,which was higher than 76.67% of the Xiaoxintong group,the difference was statistically significant(P<0.05).After treatment,the duration of both groups were shorter than those before treatment,and the onset frequency and Nitroglycerin dosage of both groups were lower than those before treatment,the duration of the self-prepared decoction group was shorter than that of the Xiaoxintong group,and the onset frequency and Nitroglycerin dosage of the self-prepared decoction group were lower than those of the Xiaoxintong group,the differences were statistically significant(P<0.05).After treatment,the levels of NT-proBNP,MPO,hs-CRP and whole blood high shear viscosity,whole blood low shear viscosity,plasma viscosity and hematocrit of both groups were lower than those before treatment,and those of the self-prepared decoction group were lower than those of the Xiaoxintong group,the differences were statistically significant(P<0.05).Conclusion:Yiqi Huoxue Bushen Decoction combined with Aspirin Enteric Capsules is effective in the treatment of unstable angina pectoris of coronary heart disease.It can regulate the expression levels of NT-proBNP,MPO,hs-CRP,improve blood rheology,and relieve symptoms of angina pectoris.
