一侧穿刺两侧分布技术在椎体成形术中的应用研究

Study on the Application of One Side Puncture and Two Sides Distribution Technique in Vertebroplasty

目的:分析一侧穿刺两侧分布技术在椎体成形术中的应用效果。方法:选取2019年5月-2021年10月于南昌市洪都中医院行经皮椎体成形术治疗的80例骨质疏松椎体压缩性骨折患者作为研究对象。采用随机数字表法分为治疗组和对照组,各40例,对照组行脊神经后支阻滞结合经皮椎体成形术治疗,治疗组行脊神经后支阻滞结合带弯角经皮椎体成形术治疗。对比两组术中出血量、手术时间、骨水泥用量、X线曝光次数及术前1 d、术后7 d、1个月的疼痛视觉模拟评分(VAS)、Oswestry功能障碍指数(ODI)评分。结果:治疗组术中出血量、骨水泥用量、X线曝光次数均少于对照组,手术时间短于对照组,差异均有统计学意义(P<0.05)。两组术前1 d的VAS、ODI评分比较,差异均无统计学意义(P>0.05);治疗组术后7 d、1个月的VAS、ODI评分均低于对照组(P<0.05)。结论:一侧穿刺两侧分布技术在椎体成形术中的应用价值显著。

Objective:To analyze the application effect of one side puncture and two sides distribution technique in vertebroplasty.Method:A total of eighty patients with osteoporotic vertebral compression fractures who underwent percutaneous vertebroplasty in Nanchang Hongdu Hospital of Traditional Chinese Medicine from May 2019 to October 2021 were selected as the research objects.They were randomly divided into treatment group and control group by random number table method,with 40 cases in each group.The control group was treated with posterior branch block of spinal nerve combined with percutaneous vertebroplasty,and the treatment group was treated with posterior branch block of spinal nerve combined with percutaneous vertebroplasty with curved angle.The intraoperative bleeding volume,operation time,bone cement dosage,number of X-ray exposures,pain visual analogue scale(VAS) and Oswestry dysfunction index(ODI) scores of 1 d before operation,7 d and 1 month after operation were compared between the two groups.Result:The amount of intraoperative bleeding,the amount of bone cement and the times of X-ray exposure in the treatment group were less than those in the control group,and the operation time was shorter than that in the control group,the differences between the groups were statistically significant(P<0.05).There were no significant differences between the two groups in VAS and ODI scores 1 d before operation(P>0.05);the VAS and ODI scores of the treatment group on the 7 d and 1 month after operation were lower than those of the control group(P<0.05).Conclusion:One side puncture and two sides distribution technique has significant application value in vertebroplasty.
