改良Karydakis手术治疗骶尾部藏毛疾病

Clinical Effect of Modified Karydakis Flap for Sacrococcygeal Pilonidal Sinus Disease

目的总结改良Karydakis手术治疗骶尾部藏毛疾病的临床疗效。方法回顾性分析2017年1月至2020年6月在我院行改良Karydakis手术治疗的22例骶尾部藏毛疾病患者的临床资料。结果本组22例均顺利完成手术,手术时间40~72 min,平均(51.18±11.63)min;拔除引流管时间为5~8 d,平均(6.01±0.88)d;住院时间6~9 d,平均(7.37±1.50)d。术后2周切口拆线。切口愈合时间11~15 d,平均(13.23±2.12)d。16例(72.73%)切除标本发现有毛发。本组病理均示藏毛窦疾病。术后切口血肿及伤口裂开1例,经门诊换药后二期愈合,无一例切口感染等发生。所有患者均获随访,随访时间12~48个月。术后1年复发1例(4.55%),再次行改良Karydakis手术,随访至术后18个月无复发。结论改良Karydakis皮瓣手术治疗骶尾部藏毛疾病操作简单、疗效可靠,患者恢复快,适合基层医院推广。

Objective To assess the therapeutic effect of the modified Karydakis flap for sacrococcygeal pilonidal disease.Method The clinical data of 22 patients suffered sacrococcygeal pilonidal disease treated by modified Karydakis operation admitted in our hospital from January 2017 to June 2020 were analyzed retrospectively.Results The operations were successful in all 22 patients.The operation time was 40 to 72 min with average of(51.18±11.63)min.The extubation time was 5 to 8 days with average of(6.01±0.88)days.The hospital stay was 6 to 9 days with average of(7.37±1.50)days.All cases were performed suture removal 2 weeks after the surgery.The healing time of wound was 11 to 15 days with average of(13.23±2.12)days.The pathological results showed pilonidal sinus disease in all cases.One case was found incision hematoma and wound dehiscence,and no incision infection occurred.All patients were followed up for 12 to 48 months.One case(4.55%)recurred one year after the operation,and underwent modified Karydakis operation again without recurrence 18 months after the re-operation.Conclusion The modified procedure of Karydakis flap is an effective technique with relatively simple operating and fast recovery for pilonidal sinus disease.
