手法挤奶联合电动吸奶器泵奶对早产母婴分离产妇泌乳的影响

Effect of manual milking combined with electric breast pump suction on lactation of puerperae separated from their premature infants

目的探讨手法挤奶联合电动吸奶器泵奶对早产母婴分离产妇泌乳量和乳房胀痛的影响。方法以2020年1月至2021年2月就诊于华中科技大学同济医学院附属同济医院产科的140名早产母婴分离产妇作为研究对象,根据随机数字表法将其分为对照组和观察组,各70名。2组产妇均给予基础护理,在此基础上,对照组产妇采用手法挤奶,观察组产妇采用手法挤奶联合电动吸奶器泵奶。比较2组产妇各个时间段的泌乳量、乳房肿胀度、乳房疼痛情况和自我效能,以及护理满意度。结果干预2d、3d后,2组的泌乳量均较干预1d后增多(P均<0.05),且观察组均多于同期对照组(P均<0.05)。干预2 d、3 d后,观察组的乳房肿胀度均轻于同期对照组(P均<0.05),视觉模拟评分法(VAS)评分均低于同期对照组(P均<0.05)。干预3 d后,2组的中文版母乳喂养自我效能量表(BSES)各维度评分均较干预前升高(P均<0.05),且观察组均较对照组更高(P均<0.05)。观察组的护理满意率(97.14%)较对照组(87.14%)更高(P<0.05)。结论手法挤奶联合电动吸奶器泵奶可有效减轻早产母婴分离产妇的乳房胀痛,增加其泌乳量,且提高自我效能和护理满意度。

Objective To explore the effect of manual milking combined with electric breast pump suction on the milk volume and breast tenderness in puerperae separated from their premature infants.Methods From January 2020 to February 2021,140 puerperae who were separated from their premature infants in the Department of Obstetrics at Tongji Hospital,Tongji Medical College,Huazhong University of Science and Technology were selected as the research objects.They were divided into control and observation groups by random number table method,with 70 cases in each group.Both groups were given basic nursing.The control group was additionally given manual milking,and the observation group was additionally given manual milking combined with electric breast pump suction.The milk volume,breast swelling,breast pain,and self-efficacy in each period,as well as the nursing satisfaction,were compared between the two groups.Results After 2 and 3 days of intervention,the milk volume in both groups were increased compared with that after 1 day of intervention(all P<0.05),and the milk volume of the observation group was greater than that of the control group(both P<0.05).After 2 and 3 days of intervention,the breast swelling degree and visual analogue scale(VAS)scores of the observation group were lower than those of the control group(all P<0.05).After 3 days of intervention,the scores of each dimension of Chinese version of the breastfeeding self-efficacy scale(BSES)in both groups were higher than those before the intervention(all P<0.05),and the scores of the observation group were higher than those of the control group(both P<0.05).The nursing satisfaction rate of the observation group(97.14%)was higher than that of the control group(87.14%,P<0.05).Conclusion Manual milking combined with electric breast pump suction can effectively relieve the breast tenderness,increase the milk production,and improve the self-efficacy and nursing satisfaction in puerperae separated from their premature infants.
