针刺治疗短暂性脑缺血发作的组穴规律研究

Study on Group Point Rule of Acupuncture in Treating Transient Ischemic Attack

目的探讨临床针刺治疗短暂性脑缺血发作的组穴应用规律。方法检索中国知网、万方、维普、PubMed电子数据库,筛选文献建立数据库。应用Excel进行描述性统计分析,运用SPSS Clementine 12.0进行关联规则分析,应用SPSS 26.0进行聚类分析。结果共纳入60个针刺处方,47个腧穴,使用频次前5位的腧穴为百会、风池、颈夹脊、大椎、风府,督脉上的腧穴最多,多集中在头颈部,重视交会穴等特定穴的选用。关联度排名前4位的腧穴组合为百会-风府、大椎-哑门、百会-哑门、百会-哑门-大椎,并将高频腧穴聚为5类。结论临床针刺治疗TIA取穴以头部为主,多采用近端取穴、循经取穴联合远端取穴的取穴原则,腧穴配伍重视调动阳气、醒脑开窍、滋补气血。

Objective To investigate the clinical application of acupuncture in the treatment of transient ischemic attack(TIA).Methods The electronic databases of CNKI,Wanfang,VIP and PubMed were searched.Excel was used for descriptive statistical analysis,SPSS Clementine 12.0 was used for association rule analysis and SPSS 26.0 was used for cluster analysis.Results A total of 60 acupuncture prescriptions were included,and 47 acupoints were selected.The top 5acupoints were Baihui(GV20),Fengchi(GB20),Xiangjiaji,Dazhui(GV14)and Fengfu(GV16).The most acupoints were on the Du meridian,mostly on the head and neck,and special acupoints such as rendezvous were selected.The top four combinations of acupoints were Baihui-Fengfu,Dazhui-Yamen(GV15),Baihui-Yamen(GV15)and Baihui-Yamen(GV15)-Dazhui,and the high-frequency acupoints were grouped into five categories.Conclusion In clinical acupuncture treatment of TIA,acupoint selection is mainly at the head,and the acupoint selection principle of proximal acupoint selection,meridian acupoint selection and distal acupoint selection is mostly adopted.Attention should be paid to activating Yang Qi,refreshing the brain and opening the orificiaries,nourishing Qi and blood.
