摘要
目的了解济南市基层医疗机构全科医生执业现状、工作压力及其满意度。方法于2020年6-9月,采用分层整群随机抽样的方法,在济南市6个区县抽取30家基层医疗机构及其169名全科医生进行问卷调查。结果调查机构平均每万人口拥有全科医生1.79人,其中社区卫生服务机构2.15人,乡镇卫生院1.51人(P<0.05);社区全科医生的平均年龄大于乡镇(P<0.05);社区工资收入高于乡镇(P<0.01);65.09%的全科医生表示压力较大和非常大;全科医生的满意度总分(37.30±7.54)分,收入、职业发展、荣誉奖励和单位福利满意度得分较低,群众认可度和人际关系满意度得分较高(P<0.01);工作压力和工作满意度呈负相关(r_(s)=-0.31,P<0.01)。结论济南市基层医疗机构全科医生队伍取得一定发展,但数量相对不足,城乡配置不均衡;全科医生工作压力较大,工作满意度中等水平。
Objective To understand the practice status,occupational stress and job satisfaction of general practitioners(GPs)in primary medical institutions in Jinan city.Methods A questionnaire survey was conducted among 30 primary medical institutions and their 169 general practitioners which were selected by using the stratified cluster random sampling method from 6 districts and counties of Jinan city from June to September,2020.Results The average number of GPs per ten thousand populations was 1.79 in institutions survey.The number of the GPs of per ten thousand populations of the community health service institutions and township health centers was 2.15 and 1.51(P<0.05),respectively.The average age of GPs in community health services was higher than that of in township health centers(P<0.05).The income of GPs in community health service institutions was higher than that of in township health centers(P<0.01).And 65.09%of GPs felt that occupational pressure was high or very high.The total score of GPs’overall satisfaction was 37.30±7.54.The satisfaction scores of income,career development,honor award and unit welfare were lower than those of other dimensions.The satisfaction scores of residents’acceptance and interpersonal relationship were higher than those of other dimensions(P<0.01).There was a negative correlation between job stress and job satisfaction(rs=-0.31,P<0.01).Conclusion The GPs in primary medical institutions had made a certain development,but the number is still insufficient and the distribution between urban and rural areas is unbalanced.The GPs in primary medical institution have heavy occupational stress and their job satisfactions are the medium level.
