摘要
目的分析MSCT联合血清脂肪酶、淀粉酶检测对重型急性胰腺炎(SAP)的诊断效能。方法收集本院2017年3月至2019年7月收治的137例急性胰腺炎患者的临床资料,其中MAP患者71例,SAP患者66例,另选取同期在本院进行体检的健康人群67例,为健康组。对患者所得MSCT图像进行观察,了解不同人群中脂肪酶、淀粉酶表达情况,并分析MSCT、血清脂肪酶、淀粉酶检测以及联合检查对SAP检出灵敏性、特异性、准确性。结果不同人群中血清脂肪酶、淀粉酶表达水平存在明显差异,脂肪酶、淀粉酶表现水平均以S.AP组最高,其次为MAP组,最低为健康组,(P<005);M.SCT对SA.P检出灵敏性.、特异性、准确性分别为7.878%、7259.09%、6212%;淀粉酶为7272%、6060%、5000%;三种检查两两之间灵敏.性、特异性、准确性比较差异无统计学意义(P>005),四种.联合检查.诊断灵敏性.、特异性、准确性分别为984.8%、9545%、10000%,明显高于单独检查(P<005)。结论MSCT联合血清脂肪酶、淀粉酶检测可提高对SAP诊断灵敏性、特异性、准确性,对患者病情进行有效评估,为治疗提供参考,对患者预后有好的影响。
Objective To analyze the diagnostic efficacy of Ms CT combined with serum lipase and amylase tests for severe acute pancreatitis(s AP). Methods The clinical data of 137 patients with acute pancreatitis treated in our hospital from March 2017 to July 2019 were collected, of which 71 were MAP patients and 66 were s AP patients. Another 67 healthy people who underwent physical examinations in our hospital during the same period were selected as the healthy group. The Ms CT images of the patients were observed to understand the expression of lipase and amylase in different people. The sensitivity, specificity and accuracy of Ms CT, serum lipase, amylase detection and combined detection in the detection of s AP were analyzed. Results There were significant differences in expression levels of serum lipase and amylase in different people. The lipase and amylase levels were highest in the s AP group, followed by the MAP group and the lowest in the healthy group(P<0.05). The sensitivity, specificity, and accuracy of Ms CT in the detection of s AP were 78.78%, 72.72%, and 81.81%, respectively. The sensitivity, specificity, and accuracy of lipases in the detection of s AP were 74.24%, 59.09%, and 62.12%, respectively. The sensitivity, specificity, and accuracy of amylases in the detection of s AP were 72.72%, 60.60%, and 50.00%. There was no significant difference in sensitivity, specificity, and accuracy between the three tests(P>0.05). The sensitivity, specificity, and accuracy of the four combined tests were 98.48%, 95.45%, and 100.00%, which were significantly higher than the single test(P<0.05). Conclusion Ms CT combined with serum lipase and amylase detection can improve the sensitivity, specificity and accuracy in the diagnosis of s AP, effectively evaluate the patient’s condition, provide a reference for treatment, and have a good impact on the patient’s prognosis.
