腕关节镜辅助钛内固定器治疗桡骨远端骨折的临床观察

Clinical Observation of Wrist Arthroscopy Assisted Titanium Internal Fixator in the Treatment of Distal Radius Fracture

目的:分析骨科桡骨远端骨折患者采取腕关节镜辅助钛内固定器治疗的可行性和临床效果,为桡骨远端骨折疾病治疗工作提供参考。方法:选择我院骨科2018年8月-2019年6月诊治的80例桡骨远端骨折患者,以随机法分为对照组(n=40)、观察组(n=40)。对照组患者采取闭合复位、石膏外固定方法治疗,观察组患者采取腕关节镜辅助钛内固定器治疗,比较2组桡骨远端骨折患者手术情况。结果:组间创伤性关节炎、关节内骨折最大间隙>1mm发生情况以及透视次数比较,观察组均明显少于对照组(P<0.05);组间腕关节背伸、掌屈活动度以及腕关节负重后疼痛评分、握力、尺桡偏、腕关节功能评分等腕关节功能恢复情况比较,观察组均优于对照组(P<0.05);组间临床疗效比较,观察组优于对照组(P<0.05);组间生活质量评分以及效果满意度比较,观察组较对照组更高(P<0.05);组间住院时间比较,观察组明显短于对照组(P<0.05)。结论:腕关节镜辅助钛内固定器治疗桡骨远端骨折整体效果明显优于闭合复位、石膏外固定法,患者术后腕关节功能恢复良好,安全性高。

Objective:To analyze the feasibility and clinical effect of wrist arthroscopy assisted titanium internal fixator in the treatment of distal radius fracture,and to provide reference for the treatment of distal radius fracture.Methods:A total of 80 patients with distal radius fracture diagnosed and treated in the department of Orthopedics of our hospital from August 2018 to June 2019 were randomly divided into control group(n=40)and observation group(n=40)•Patients in the control group were treated with closed reduction and plaster external fixation,while patients in the observation group were treated with wrist arthroscopy assisted with titanium internal fixator.The surgical conditions of patients with distal radius fractures in the two groups were compared.Results:The incidence of traumatic arthritis,the maximum space of intra articular fracture>1 mm and the number of fluoroscopy in the observation group were significantly less than that in the control group(P<0.05).The results showed that the recovery of wrist function in the observation group was better than that in the control group(P<0.05).The observation group was better than the control group(P<0.05).Comparison of quality of life score and effect satisfaction between groups showed that the observation group was higher than the control group(P<0.05).The length of hospitalization in observation group was significantly shorter than that in control group(P<0.05).Conclusion:The overall effect of wrist arthroscopy-assisted titanium internal fixator in the treatment of distal radius fracture is significantly better than that of closed reduction and plaster external fixation,and the patients have good postoperative wrist joint function recovery and high safety.
