MRI对膝关节半月板和关节软骨损伤的临床诊断价值

The Clinical Diagnostic Value of MRI in Knee Meniscus Injury and Knee Cartilage Injury

目的:探究核磁共振成像(Magnetie Resonance Imaging,MRI)应用于膝关节半月板损伤(Knee.Meniseus Injure,KMI)和膝关节软骨损伤(Knee Cartilage Injure,KCI)诊断中的临床价值。方法:从2019年1月1日-2020年6月30日在中铁十九局集团有限公司职工中心医院就诊的58例疑似KMI和KCI患者纳入本次的研究范围,对所有患者均进行CT、MRI、关节镜检查,以关节镜诊断结果为金标准,计算CT、MRI的诊断准确率、敏感度、特异度,观察不同等级的确诊患者的CT、MRI诊断准确情况。结果:MRI诊断膝关节半月板损伤和关节软骨损伤的准确率为91.38%,敏感度为97.78%,特异度为63.64%。MRI诊断0级7例,准确率为87.50%;MRI诊断Ⅰ级15例,准确率为83.33%;MRI诊断Ⅱ级24例,准确率为96.00%;MRI诊断Ⅲ级7例,准确率为100.00%。MRI诊断结果与关节镜诊断结果差异无统计学意义(P>0.05);MRI诊断膝关节半月板损伤的诊断结果与CT诊断结果差异有统计学意义(P<0.05)。结论:MRI在KMI和KCI中具有比较高的诊断价值,诊断的准确率、灵敏度、特异度均比较高,除此之外还能准确地对患者的病情进行分级,为治疗提供准确的影像学依据,有利于患者的预后。

Objective:To explore the clinical value of magnetic resonance imaging(MRI)in the diagnosis of knee meniscus injury(KMI)and knee cartilage injury(KCI).Methods:During from January 1,2019 to June 30,2020,a total of 58 patients with suspected KMI and KCI treated in the Staff Central Hospital of China Railway 19th Bureau Group Co.,Ltd.were included in the scope of this study.All patients were examined by CT,MRI and arthroscopy.The diagnostic accuracy,sensitivity and to observe the diagnostic accuracy of CT and MRI in patients with different levels of diagnosis.Results:The accuracy,sensitivity and specificity of MRI in the diagnosis of knee meniscus injury and articular cartilage injury were 91.38%,97.78%and 63.64%.MRI diagnosed grade 0 in 7 cases,and the accuracy was 87.50%;15 cases of grade Ⅰ were diagnosed by MRI,and the accuracy rate was 83.33%;24 cases of grade Ⅱ were diagnosed by MRI,and the accuracy rate was 96.00%;7 cases of grade Ⅲ were diagnosed by MRI,and the accuracy rate was 100.00%.There was no significant difference between MRI and arthroscopy(P>0.05);There was significant difference between MRI and CT in the diagnosis of knee meniscus injury(P<0.05).Conclusion:MRI has high diagnostic value in knee meniscus injury and articular cartilage injury.The diagnostic Accuracy,sensitivity and specificity are relatively high.In addition,MRI can accurately grade the patients condition,provide accurate imaging basis for treatment,and is conducive to the patients prognosis.
