多发性骨髓瘤化疗患者生活质量现状调查及其影响因素分析

Investigation of life quality in patients with multiple myeloma transplantation and analysis of its influencing factors

目的研究多发性骨髓瘤(Multiple Myeloma,MM)化疗患者的生活质量现状及其影响因素。方法选择2020年1月~2021年6月在我院接受化疗的230例多发性骨髓瘤患者为研究对象,采用中文版欧洲癌症研究与治疗组织生活质量问卷(Euro-pean Organization for Researchand Treatment of Cancer quality of life questionnaire,EORTC QLQ-C30)对患者进行调查,统计并分析其主要危险因素。结果MM化疗患者的躯体功能、角色功能、社会功能、总体健康状况、食欲下降、便秘、经济困难与常模相比,差异均有统计学意义(均P<0.05)。单因素方差分析发现,年龄、职业、因病中断工作、收入、血红蛋白、血肌酐、β_(2)-微球蛋白骨髓中浆细胞系异常增生比例为MM化疗患者生活质量的影响因素,差异均具有统计学意义(均P<0.05);经多元逐步回归分析发现,MM化疗患者生活质量的独立危险因素包括:身体功能状况、事情变化的适应能力、没希望依然不放弃(均P<0.05)。结论多发性骨髓瘤化疗患者的生活质量水平较低,年龄、职业、因病中断工作等都会是其生活质量水平下降的影响因素。

Objective To investigate the status of quality of life in patients with multiple myeloma transplantation and its influencing factors.Methods A total of 230 patients with multiple myeloma receiving chemotherapy in our hospital from January 2020 to January 2021 were selected as the research subjects.The Euro-Pean Organization for Research and Treatment of Cancer Quality of Life Questionnaire(EORTC QLQC-30),the Chinese version,was used to investigate the main risk factors of the patients.Results The physical function,role function,social function,general health status,decreased appetite,constipation and economic difficulties of MM chemotherapy patients were compared with the norm,and the differences were statistically significant(P<0.05 for each).One-way ANOVA showed that age,occupation,interruption of work due to disease,income,abnormal hyperplasia of plasma cell lines in bone marrow of hemoglobin,serum creatinine andβ_(2)-microglobulin were the influencing factors of quality of life in MM chemotherapy patients,and the difference was statistically significant(P<0.05 for each).Multiple stepwise regression analysis showed that independent risk factors for QoL in patients with MM chemotherapy included functional status,ability to adapt to changes in events,and not giving up when there was no hope(P<0.05 for each).Conclusion The quality of life of patients with multiple myeloma transplantation was lower,and age,occupation,and interruption of work due to disease were all factors affecting the quality of life.
