续断接骨汤辅助髋关节置换术治疗股骨颈骨折的效果评价

Effect evaluation of Xuduanjiegu Decoction assisted hip arthroplasty in the treatment of femoral neck fracture

目的探讨续断接骨汤辅助髋关节置换术治疗股骨颈骨折的效果。方法选择2019年12月~2021年3月在我院行髋关节置换术的股骨颈骨折患者40例为研究对象,按照随机数字表法分为两组,每组20例。对照组行髋关节置换术联合口服钙尔奇D片治疗,观察组在对照组的基础上联合续断接骨汤治疗,观察比较两组患者的临床症状缓解情况(疼痛消退时间、肿胀消退时间)、生化指标水平[骨碱性磷酸酶(bone alkaline phosphatase,BALP)、尿脱氧吡啶酚(urinary deoxypyridinoline,DPD)、抗酒石酸酸性磷酸酶5b(tartrate-resistant acid phosphatase 5b,TRACP5b)]、骨细胞因子水平[骨形态发生蛋白(bone morphogenetic protein,BMP)、成纤维细胞生长因子(fibroblast growth factor,FGF)、转化生长因子-β(transforming growth factor-β,TGF-β)]以及Harris评分。结果观察组的疼痛消退时间、肿胀消退时间均短于对照组,差异均具有统计学意义(均P<0.05)。两组患者治疗后的BALP水平均高于治疗前,DPD、TRACP5b水平均低于治疗前,且观察组各指标水平改善均优于对照组,差异均有统计学意义(均P<0.05)。两组患者的骨细胞因子水平比较,治疗后的BMP、FGF、TGF-β水平两组均高于治疗前,且观察组各指标水平均高于对照组(均P<0.05)。两组患者的Harris评分比较,观察组明显高于对照组,差异有统计学意义(P<0.05)。结论续断接骨汤辅助髋关节置换术治疗股骨颈骨折的效果好,可明显缓解患者的临床症状、促进关节活动功能恢复。

Objective To investigate the effect of Xuduanjiegu Decoction assisted hip arthroplasty in the treatment of femoral neck fracture.Methods 40 patients treated in our hospital from December 2019 to March 2021 were included as the research object.According to the random number table method,they were divided into two groups,with 20 cases in each group.Control group:hip arthroplasty combined with oral calcium Erqi D tablets.Observation group:on the basis of the control group,combined with Dipsacus Jiegu decoction.Observe the relief of clinical symptoms(pain relief time)swelling regression time,biochemical indexes,including bone alkaline phosphatase(BALP),urinary deoxypyridinoline(DPD),tartrate resistant acid phosphatase 5 b(Tracp5b)and bone cytokines,including bone morphogenetic protein(BMP),fibroblast growth factor(FGF),transforming growth factor-β(transforming growth factor-β,TGF-β)Level,Harris score.Results The pain subsided time and swelling subsided time in the observation group were earlier than those in the control group,and there were significant difference between the data(P<0.05 for both);After the intervention,the BALP level of the observation group was higher than that of the pre intervention and control groups,and the levels of DPD and Tracp5b were lower than those of the pre intervention and control groups.The difference between the data were significant(P<0.05 for each);the level of bone cytokines in the observation group after intervention was higher than that before intervention and the control group,and there was significant difference between the data(P<0.05);the Harris score of the observation group after intervention was higher than that before intervention and the control group,and the difference between the data was significant(P<0.05).Conclusion Xuduanjiegu Decoction assisted hip replacement has a good effect in the treatment of femoral neck fracture,which can significantly alleviate the clinical symptoms of patients and promote the recovery of joint activity function.
