超声引导下高强度聚焦超声消融术治疗子宫肌瘤的有效性及安全性研究

Efficacy and safety of ultrasound-guided high-intensity focused ultrasound ablation for uterine fibroids

目的分析子宫肌瘤治疗中超声引导下高强度聚焦超声消融术的有效性及安全性。方法选取120例子宫肌瘤患者,按照入院顺序排号,根据奇偶数进行分组,偶数组为研究组,奇数组为对照组,每组60例。研究组实施超声引导下高强度聚焦超声消融术治疗,对照组实施腹腔镜下子宫肌瘤剔除术治疗。对比两组手术相关指标、并发症发生情况,观察研究组患者手术前、手术后3、6、12个月病灶体积变化情况。结果两组手术时间对比,差异无统计学意义(P>0.05),研究组术中无出血量,术后自主活动时间及住院时间均明显短于对照组,差异有统计学意义(P<0.05)。研究组术后并发症发生率为5.00%,明显低于对照组的16.67%,差异有统计学意义(P<0.05)。研究组患者手术后3、6、12个月病灶体积较手术前显著缩小,差异有统计学意义(P<0.05)。结论子宫肌瘤患者实施超声引导下高强度聚焦超声消融术效果确切,可有效缩小病灶体积,且安全性较高,具有临床推广价值。

Objective To analyze the effectiveness and safety of ultrasound-guided high-intensity focused ultrasound ablation in the treatment of uterine fibroids.Methods 120 patients with uterine fibroids were selected,and they were divided into groups according to the order of admission.The even group was the research group,and the odd group was the control group,with 60 cases in each group.The study group was treated with ultrasound-guided high-intensity focused ultrasound ablation,while the control group was treated with laparoscopic myomectomy.Surgical indicators and complications were compared between the two groups,and the changes of lesion volume before and 3,6 and 12 months after surgery were observed in the study group.Results There was no significant difference in the operative time between the two groups(P>0.05).There was no intraoperative blood loss in the study group,and the postoperative autonomous activity time and hospitalization time were significantly shorter than those in the control group,with statistically significant differences(P<0.05).The incidence of postoperative complications in the study group was 5.00%,which was significantly lower than that in the control group(16.67%),with statistical significance(P<0.05).The volume of lesions in the study group was significantly reduced 3,6 and 12 months after surgery compared with that before surgery,with statistical significance(P<0.05).Conclusion Ultrasound-guided high-intensity focused ultrasound ablation for patients with uterine fibroids is effective and can effectively reduce the volume of lesions,with high safety and clinical promotion value.
