腹腔镜与开腹手术行子宫肌瘤剔除术的效果对比

Comparison of the effect of laparoscopic and open myomectomy

目的研究对比腹腔镜与开腹手术行子宫肌瘤剔除术的临床效果。方法选取30例子宫肌瘤患者,采用随机数字表法分为对照组与研究组,各15例。对照组患者行开腹子宫肌瘤剔除术治疗,研究组患者行腹腔镜下子宫肌瘤剔除术治疗。对比两组治疗前后卵泡刺激素(FSH)、促黄体生成素(LH)水平、围术期指标及术后并发症发生情况。结果手术前,两组FSH、LH水平对比,差异无统计学意义(P>0.05);手术后,两组FSH、LH水平均高于手术前,且试验组明显高于对照组,差异具有统计学意义(P<0.05)。研究组术中出血量明显少于对照组,手术时间、排气时间、下床时间及住院时间均明显短于对照组,差异有统计学意义(P<0.05)。研究组术后并发症发生率为6.67%,明显低于对照组的46.67%,差异有统计学意义(P<0.05)。结论相比开腹手术,腹腔镜下子宫肌瘤剔除术可改善患者机体激素水平,减轻机体损伤,减少并发症,具有临床推广价值。

Objective To compare the clinical effect of laparoscopic myomectomy with laparotomy.Methods 30 patients with uterine fibroids were selected and divided into control group and study group with 15 cases in each group by random number table method.Patients in the control group were treated with open myomectomy,while patients in the study group were treated with laparoscopic myomectomy.The levels of follicle stimulating hormone(FSH)and luteinizing honnone(LH),perioperative indexes and postoperative complications were compared between the two groups before and after treatment.Results Before surgery,there was no significant difference in FSH and LH levels between the two groups(P>0.05).After surgery,the levels of FSH and LH in both groups were higher than before,and the experimental group was significantly higher than the control group,with statistical significance(P<0.05).The amount of intraoperative blood loss in the study group was significantly less than that in the control group,and the operation time,exhaust time,time to get out of bed and hospital stay were significantly shorter than those in the control group,with statistical significance(P<0.05).The incidence of postoperative complications in the study group was 6.67%,which was significantly lower than 46.67% in the control group,and the difference was statistically significant(PV0.05).Conclusion Compared with open surgery,laparoscopic myomectomy can improve the body hormone level of patients,reduce body injury,reduce complications,and has clinical promotion value.
