摘要
目的对2016—2021年分离自北京市海淀区腹泻患者的致泻大肠埃希菌(DEC)耐药结果进行分析,掌握海淀区DEC耐药情况。方法对2016—2021年收集来自3家哨点医院的195株DEC菌株进行13种抗生素的耐药试验,试验方法采用微量肉汤稀释法。结果耐药率前3位的依次为氨苄西林67.18%(131/195),萘啶酸63.08%(123/195)和头孢挫林44.62%(87/195),耐药率低于10%的为亚胺培南0.51%(1/195)、头孢他啶1.54%(3/195)、头孢西丁1.54%(3/195)和氯霉素9.23%(18/195);年份、DEC型别、是否60岁及以上年龄组和是否产超广谱β-内酰胺酶(ESBLs)均为某些抗生素耐药率的影响因素;多重耐药株占总菌株数的54.36%(106/195),最常出现的多重耐药谱为氨苄西林-头孢噻肟-头孢挫林(AMP-CTX-CFZ)。结论北京市海淀区DEC耐药情况十分严峻,改变现阶段抗生素的使用方式,减缓细菌耐药性的发展和传播迫在眉睫。
Objective The aim of this study was to investigate the antimicrobial resistance patterns of diarrheagenic Escherichia coli(DEC)isolated from diarrhea patients in Haidian District of Beijing from 2016 to 2021.Methods A total of 195 DEC strains were collected from 3 hospitals.Antimicrobial susceptibility was tested using broth microdilution method.Results The top three with the most serious antimicrobial resistance were ampicillin,nalidixic acid and cefaclor,with there antimicrobial resistance rates were 67.18%(131/195),63.08%(123/195)and 44.62%(87/195),respectively.The antimicrobial resistance rates of imipenem,ceftazidime,cefoxitin and chloramphenicol were lower than 10%,with there antimicrobial resistance rates were 0.51%(1/195),1.54%(3/195),1.54%(3/195)and 9.23%(18/195).Year,Dec type,age group(≥60 years old and<60 years old)and extended-spectrumβ-Lactamases(ESBLs)-producing were the factors affecting the antimicrobial resistance rates of some drugs.Among the 195 DEC strains 106(54.36%)were multidrug resistant.The most common antimicrobial resistance spectrum was ampicillin-cefotaxime-ceftazidime(AMP-CTX-CFZ).Conclusion The antimicrobial resistance of Dec in Beijing Haidian District was very serious.It is urgent to change the use of antibiotics and slow down the development and spread of bacterial antimicrobial resistance.
