张绚邦教授治疗脑血管疾病用药规律研究

Data mining research on medication rules of professor Zhang Xuanbang in treatment of cerebrovascular diseases

目的基于聚类分析和因子分析探讨张绚邦教授治疗脑血管疾病用药特色及用药规律,为中医药治疗脑血管疾病提供参考。方法回顾性分析张绚邦教授1996年6月-2000年1月在新疆维吾尔自治区中医医院门诊治疗脑血管疾病所开处方,构建处方数据库,所有中药分别由2人输入至2016版Excel表中,进行性味、归经、功效分析,将高频中药导入SPSS Statistics 25.0软件进行聚类分析和因子分析,导入SPSS Modeler 18.0软件进行关联规则分析,找寻核心药对和药组。结果439首处方涉及218味中药,使用频数最高的为珍珠母(342次,77.90%),药性微寒平和,药味以甘、苦、辛为主,主要归肝、心、肺经,多以决明子-石决明、石决明-决明子-珍珠母、石决明-决明子-钩藤等配伍,核心药对7对。结论张绚邦教授治疗脑血管疾病用药以平肝熄风、活血化瘀、安神为主,多以明珠钩藤方为基本方,辅以化痰除烦、清心润肺、强筋通络、清肝明目之法。

Objective To analyze and mine the medication characteristics and medication rules of Zhang Xuanbang in the treatment of cerebrovascular diseases based on factor analysis and cluster analysis,so as to provide the reference for treatment of cerebrovascular diseases with traditional Chinese medicine(TCM).Methods Collecting the prescriptions prescribed by Zhang Xuanbang for treatment of cerebrovascular diseases in the hospital from June 1996 to January 2000 and constructing the prescription database.All drugs were input into excel 2016 by 2 related researchers respectively for property,flavor,meridian and efficacy analysis.The high-frequency drugs were imported into SPSS sta tistics 25.0 for cluster analysis and factor analysis,then into SPSS modeler 18.0 for association rule analysis to mine the core drug pairs and groups.Results 439 prescriptions involved 218 TCM.The Mother of Pearl(342 times,77.90%)was the most frequently used.The drug was medium cool and mild with sweet,bitter and pungent flavor,main⁃ly belonging to liver,heart and lung meridians.Drug pairs included Cassia seed-Concha Haliotidis,Concha Haliotidis-Cassia seed-Mother of pearl,Concha Haliotidis-Cassia seeduncaria stem with hooks,etc.The core drugs were grouped into 7 group.Conclusion Zhang Xuanbang's medication for treatment of cerebrovascular diseases mainly include calm liver and wind,and to promote blood circulation,and to remove blood stasis and to calm mind.Most of doctors take Mingzhu Gouteng formula as the basic formula,supplemented by the methods of resolving phlegm and removing annoyance,clearing the heart and lungs,strengthening tendons and collaterals,clearing the liver and brightening eyes.
