精细化管理对外侧单髁置换术治疗膝外侧间室骨关节炎患者的应用研究

Application of Refined Management of Lateral Mono-condylar Replacement in the Treatment of Lateral Compartment Osteoarthritis of the Knee

目的:研究对膝外侧间室骨关节炎患者行外侧单髁置换术后采用精细化管理的临床效果。方法:选取2018年12月-2020年12月我院接受治疗的骨关节炎患者260例作为研究对象,将所有患者随机分为观察组和对照组,每组130例。对照组术后采用常规护理,观察组术后采用精细化管理,观察2组患者护理前后的视觉模拟疼痛(VAS)评分及膝关节功能(Lysholm)评分情况,对2组患者术后分别进行回访,观察其术.后7天、15天、30天的膝关节损伤和骨关节炎结果(K00S)评分情况,以判定其远期疗效,并对患者进行满意度调查。结果:观察组经护理后其VAS评分及Lysholm评分均优于对照组,经对比差异具有统计学意义(P<0.05),通过对2组患者进行回访后发现观察组术后7天、15天、30天的KOOS评分均优于对照组,且观察组的满意率为97.69%(127/130),比对照组的76.92%(100/130)高,以上数据经对比差异均具有统计学意义(P均<0.05)。结论:对行外侧单髁置换术的膝外侧间室骨关节炎患者采用精细化管理,可有效缓解患者术后的疼痛程度并加快恢复其膝关节功能,且患者经护理后其远期疗效更高,对护理工作的满意度更高。精细化管理临床价值较高,建议予以推广应用。

Objective:To study the clinical effect of fine management after lateral mono-condylar replacement in patients with lateral compartment osteoarthritis of the knee.Methods:Selection in December 2018 in December 2020 to the hospital for treatment during the period of 260 patients with osteoarthritis as the research object,all of the patients were randomly divided into observation group and control group,130 cases in each group,control group using conventional postoperative nursing,postoperative observation group USES the fine management,observe two groups before and after patient care visual analog pain score(VAS)and knee function(Lysholm)score,respectively on two groups of patients with postoperative visit,observe the postoperative 7 d,15 d,30 d of knee injury and osteoarthritis outcome(KOOS)score,In order to detemiine its long-tenn efficacy and patients satisfaction survey.Results:Observation group after nursing the VAS score and the Lysholm scores were better than control group,the contrast differences statistically significant(P<0.05),by means of two groups of patients after comming back found the group of 7 d,15 d,30 d after KOOS score were superior to control group,and group the satisfaction rate of 97.69%(127/130)is higher than the control group 76.92%(100/130),the above data by the comparative differences are statistically significant(P<0.05).Conclusion:Refined management of OA patients undergoing lateral mono-condylar replacement in the lateral compartment of the knee can effectively relieve postoperative pain and accelerate the recovery of knee function,and the patients have higher long-tenn efficacy and satisfaction with nursing work after nursing.The fine management has high clinical value,so it is recommended to be popularized and applied.
