先天性多拇畸形纠正轴线偏斜方法的研究进展

先天性多拇畸形是最常见的手足部畸形,复杂的多拇整复术可能需要增容、纠偏、减数、短缩等多个方面。其中纠正指体轴线偏斜往往是技术难点,同时成角畸形是术后最常见的遗留或继发畸形。本研究回顾复杂型多拇整复相关文献,介绍造成多拇轴线偏斜的病理因素;结合作者的经验,重点详述整复术中纠正轴线偏斜方法的研究进展,包括截骨、关节成形和韧带紧缩、重建拇短展肌止点、肌腱中央化和重平衡等。

Congenital thumb duplication is the most common deformity of distal extremities.Surgical reconstruction of complex thumb duplication may include augmentation,correcting axial deviation,number reduction and length shortening.Correcting angular deformation of thumb has been one technical challenge.Angular deformation is one of the most common surgical complications in the literature.This review introduced the pathological factors of angular deformation in thumb duplication and summarized the improved methods of correcting deformations,including osteotomy,arthroplasty,ligament tightening,adductor pollicis brevis reattachment,tendon centralization and rebalance,etc.
