首发单相与双相抑郁障碍患者局部脑区静息态功能磁共振低频振幅对比观察 被引量:5

Comparative observation of resting-state amplitude of low-frequency fluctuation in local brain regions of patients with first-onset unipolar and bipolar depression

摘要 目的观察以抑郁症状为首发症状的单相抑郁障碍(UD)与双相抑郁障碍(BD)患者局部脑区静息态功能磁共振成像(rs-fMRI)中低频振幅(ALFF)的差异。方法选取25例UD患者(UD组)、20例处于抑郁发作的BD患者(BD组)及25例健康对照者(健康对照组),对所有受试者进行rs-fMRI扫描,计算全脑ALFF值,分析不同脑区ALFF差异。结果三组间ALFF值有统计学差异的脑区为右额中回、左额上回、左梭状回、左脑岛、右豆状壳核、左小脑、左海马;与健康对照组比较,UD组在左额中回、右颞上回及左颞中回的ALFF值升高,BD组在右额中回、右额上回、左额中回、左小脑、左海马、左脑岛及右豆状壳核的ALFF值升高;与UD组比较,BD组在左额上回、左后扣带回、左楔前叶、左额中回及左海马的ALFF值升高(P均<0.01)。结论UD与BD患者ALFF值在左额上回、左后扣带回、左楔前叶、左额中回及左海马有差异,这些脑区的活动改变有助于早期鉴别UD、BD患者。 Objective To observe the difference in amplitude of low-frequency fluctuation(ALFF)in resting-state functional magnetic resonance imaging(rs-fMRI)in local brain regions between first-onset unipolar depression(UD)and bipolar depression(BD)patients.Methods Twenty-five patients with unipolar depression(UD group),20 patients with bipolar depression(BD group),and 25 healthy controls(HC group)were selected.All research participants were scanned with rs-fMRI to obtain the ALFF.The difference of ALFF in different brain regions were analyzed.Results The brain regions with significant difference in ALFF values among the three groups were right middle frontal gyrus,left superior frontal gyrus,left fusiform gyrus,left insula,right lenticular putamen,left cerebellum,and left hippocampus.Compared with the healthy control group,the ALFF values in the left middle frontal gyrus,right superior temporal gyrus and left middle temporal gyrus increased in the UD group,and the ALFF values in the right middle frontal gyrus,right superior frontal gyrus,left middle frontal gyrus,left cerebellum,left hippocampus,left insula and right lenticular putamen increased in the BD group(all P<0.01).Compared with the UD group,the ALFF values in left superior frontal gyrus,left posterior cingulate gyrus,left precuneus,left middle frontal gyrus and left hippocampus increased in the BD group(all P<0.01).Conclusions The ALFF values in left superior frontal gyrus,left posterior cingulate gyrus,left precuneus,left middle frontal gyrus and left hippocampus are different between first-onset UD and BD patients.The activity changes in these brain regions can be helpful to early identify UD and BD patients.
