2型糖尿病患者用药依从性的干预研究评价

Evaluation of an intervention study on medication adherence in patients with type 2 diabetes

目的:评价以临床药师主导的2型糖尿病患者用药宣教服务对用药依从性的干预结果,为建立社区慢病管理模式提供实践依据。方法:采取自身对照研究方法,对30例2型糖尿病患者实践规范用药宣教服务(12个月以上),统计分析干预前后患者在用药依从性、药物认知度、血糖控制水平等方面变化情况。结果:患者用药依从性得分、药物认知度得分以及血糖达标率在干预后均得到提高(均P<0.05)。结论:规范建立以临床药师主导的药学服务利于2型糖尿病患者的长期慢病控制;未来应对接三级综合与社区医院的慢病管理模式,以提高及稳固患者用药依从性。

Objective:To evaluate the results of a clinical pharmacist-led medication education service for patients with type 2 diabetes on medication adherence and to provide a practice basis for establishing community-based chronic disease management.Methods:A self-control study was conducted to practice the standardized medication education service(more than 12 months)for 30 patients with type 2 diabetes,and the changes in medication adherence,medication awareness and blood glucose control level during the intervention were statistically analyzed.Results:Patients’medication adherence scores,medication awareness scores,and blood glucose compliance rates were all improved after the intervention(all P<0.05).Conclusion:The establishment of a standardized pharmacy service led by clinical pharmacists is of benefit to long-term chronic disease control in patients with type 2 diabetes.The chronic disease management modes in tertiary comprehensive and community hospitals should be connected in the future to improve and stabilize patients’medication adherence.
