摘要
目的探讨极速脉搏波技术评价慢性肾脏病(CKD)患者早期动脉硬化的应用价值。方法选取2018年1月至2020年7月宁波大学医学院附属医院肾内科住院患者401例,其中肾功能异常328例纳入病例组,肾功能正常73例纳入对照组。收集并分析2组患者一般资料、颈动脉内膜-中膜厚度、收缩起始时脉搏波传导速度(PWVBS)和收缩末期脉搏波传导速度(PWVES),计算肾小球滤过率(GFR),并分析GFR与PWVBS、PWVES的相关性。结果2组患者性别、年龄、身高、体重、收缩压、总胆固醇、低密度脂蛋白胆固醇、心脏射血分数、颈动脉内膜-中膜厚度比较,差异均无统计学意义(P>0.05);舒张压、尿酸、肌酐、尿素氮、高密度脂蛋白胆固醇、GFR、PWVBS、PWVES比较,差异均有统计学意义(P<0.05)。PWVBS、PWVES与GFR呈负相关(r=-0.568、-0.571,P<0.05)。结论极速脉搏波技术可准确评价CKD患者早期动脉硬化,可为临床早期预防CKD患者动脉硬化提供有力帮助。
Objective To evaluate the application value of ultrafast pulse wave velocity(UFPWV)technology in evaluating early cervical atherosclerosis in the patients with chronic kidney disease(CKD).Methods A total of 401 inpatients in the department of nephrology,admitted to the Affiliated Hospital of Medical School NingBo University from January 2018 to July 2020 were selected.Among them,a total of 328 cases with abnormal renal function were included in the case group,and 73 cases with normal renal function were included in the control group.The general data,carotid intima-media thickness(IMT),pulse wave velocity at the beginning of the systole(PWVBS)and pulse wave velocity at the end of the systole(PWVES)of the two groups were collected and analyzed.The glomerular filtration rate(GFR)was calculated,and the correlation between GFR and PWVBS and PWVES was analyzed.Results There were no significant differences between the two groups in gender,age,height,weight,systolic blood pressure,total cholesterol,low-density cholesterol,EF and IMT(P>0.05).There were significant differences between the two groups in diastolic blood pressure,uric acid,creatinine,urea nitrogen,high-density lipoprotein,GFR,PWVBS and PWVES(P<0.05).There were the negative correlation between the GFR and PWVBS,PWVES(r=-0.568,-0.571,P<0.05).Conclusion The UFPWV can accurately evaluate the early sclerosis of the neck arteries in the CKD patients,and provide strong help for the prevention of atherosclerosis in early clinical settings.
