脓毒症患者气管插管后低血压危险因素分析

Risk Factors of hypotension in patients with sepsis after endotracheal intubation

目的 研究脓毒症患者行气管插管后出现低血压的危险因素。方法 收集本院急救重症监护病房2018年6月-2021年7月128例确诊脓毒症并行气管插管患者的临床资料,将患者分为血压下降组和血压未下降组,首先对两组患者进行单因素分析,得出导致血压下降组的危险因素,然后再进行Logistic回归分析,最终得出导致血压下降的危险因素及其相关性。结果 共收集128例患者的完整临床资料,39例患者出现血压下降,进行单因素分析得出体重、插管前收缩压、pH值、APACHEⅡ评分、乳酸5个因素是有意义的,Logistic回归分析后体重、插管前收缩压、乳酸3个因素为导致低血压的独立危险因素。结论 对脓毒症患者行气管插管前要尽量避免低血压的发生。

Objective To study the risk factors of postintubation hypotension(PIH)in patients with sepsis. Methods The clinical data of 128 patients with sepsis and endotracheal intubation in emergency intensive care unit of our hospital from June 2018 to July 2021were collected and analyzed. The patients were divided into two groups: the decreased blood pressure group and non decreased blood pressure group. Firstly, the risk factors of the decreased blood pressure group were obtained by univariate analysis, and finally logistic regression analysis was carried out to obtain the risk factors and correlation of the decreased blood pressure. Results The complete clinical data of 128 patients were collected. 39 patients had decreased blood pressure. Univariate analysis showed that weight, systolic blood pressure before intubation, p H value, ApacheⅡscore and lactic acid were significantly important. Logistic regression analysis showed that weight, systolic blood pressure before intubation and lactic acid were independent risk factors for hypotension. Conclusion Hypotension should be avoided before endotracheal intubation in patients with sepsis.
