不同术式对肩胛骨骨折患者的疗效比较

Comparison of different types surgeries in patients with scapular fractures

目的 比较两种手术治疗肩胛骨骨折的临床疗效。方法 选取2012年6月-2016年7月我科治疗的肩胛骨骨折患者45例作回顾性分析,依据患者手术方法的不同将患者分为改良组23例与传统组22例,其中传统组实施传统Judet手术入路治疗,改良组实施改良Judet手术入路治疗。两组患者进行12个月的随访,采用Hardegger肩关节功能评价,对比两组患者治疗效果。结果 改良组患者治疗优良率91.30%,传统组治疗优良率38.10%,改良组的治疗优良率明显高于传统组,差异有统计学意义(P <0.05)。治疗后12个月随访检查结果显示,改良组平均外展肌力、关节外展活动度均高于传统组,疼痛评分、手术时间、术中出血量、手术切口低于传统组,差异有统计学意义(P <0.05)。结论 与传统Judet手术相比,改良Judet手术入路治疗肩胛骨骨折的疗效理想,该手术类型可减轻患者病痛,促进肩关节功能恢复,手术创伤小,恢复时间快,临床应用安全有效。

Objective To compare the clinical effect of two kinds of surgery on scapular fracture. Methods From June 2012 to July2016,45 patients with scapular fracture treated by our department were retrospectively analyzed. According to the different patient surgical methods, patients were divided into 23 cases of modified group and 22 cases of traditional group. Among them, the traditional group was treated with?the traditional Judet surgical approach, A modified Judet surgical approach was performed in the modified group. The two groups were followed up for 12 months, Hardegger shoulder function evaluation was used to compare?therapeutic effect of the two groups.Results The excellent and good rate of treatment the improved group was 91.30, the traditional group was 38.10. The excellent and good rate of treatment the improved group was significantly higher than that the traditional group, and the difference was statistically significant(P < 0.05).The results of follow-up examination at 12 months after treatment showed that the mean abduction muscle strength and joint abduction activity of the improved group were higher than the traditional group, and the pain score, surgical time, intraoperative bleeding and surgical incision were lower than the traditional group, statistically significant(P < 0.05). Conclusion The modified Judet surgical approach is ideal for the treatment of scapular fracture compared with traditional Judet surgery. The type of operation can alleviate the pain of patients and promote the recovery of shoulder joint function, light?surgical trauma, fast recovery time, and the clinical application is safe.
