缬沙坦氨氯地平对老年高血压患者血管弹性功能与血压变异性的影响

Effects of valsartan amlodipine on vascular elasticity and blood pressure variability in elderly patients with hypertension

目的探讨缬沙坦氨氯地平治疗老年高血压患者的临床效果。方法选择2018年2月-2020年3月就诊于河南信合医院心内科的90例老年高血压患者,按随机数字表法分为研究组和对照组,各45例。对照组口服苯磺酸氨氯地平片,研究组口服缬沙坦氨氯地平片,连续治疗6个月。比较两组临床疗效、血压变异性、血管弹性功能。结果研究组治疗总有效率较对照组高,24 h平均舒张压、24平均收缩压、24 h舒张压变异系数、24 h收缩压变异系数较对照组低,差异有统计学意义(P<0.05);研究组小动脉弹性指数(C2)、内皮依赖性血管舒张功能(FMD)、大动脉弹性指数(C1)较对照组高,心踝血管指数(CACI)、颈股动脉脉搏波传导速度(cfPWV)较对照组低,差异有统计学意义(P<0.05)。结论缬沙坦氨氯地平可降低老年高血压患者的血压水平,改善血管弹性功能与血压变异性,促进病情恢复。

Objective To investigate the clinical effect of valsartan amlodipine on elderly patients with hypertension.Methods From February 2018 to March 2020,90 elderly patients with hypertension were selected and divided into study group and control group by random number table method,45 cases in each group.Control group were given with oral amlodipine benzenesulfonic acid tablets,Study group were treated with oral valsartan amlodipine tablets.Treatment was continued for 6 months.The clinical efficacy,blood pressure variability and vascular elasticity were compared between the two groups.Results The total efficiency of the study group was higher than that of the control group,mean diastolic blood pressure in 24h,mean systolic blood pressure in 24h,variation coefficient of diastolic blood pressure in 24h,variation coefficient of systolic blood pressure in 24h were lower than those in the control group,and differences were statistically significant(P<0.05).The arteriolar elasticity index(C2),endothelium-dependent vasodilation function(FMD)and arterial elasticity index(C1)in the study group were higher than those in the control group,The myocardial vascular index(CACI)and pulse wave velocity(cf PWV)of carotid artery in the study group were lower than those of control group,difference was statistically significant(P<0.05).Conclusion Valsartan amlodipine can reduce blood pressure in elderly patients with hypertension,improve vascular elasticity and blood pressure variability and promote recovery.
