改良式高嵌体与全冠修复治疗牙髓坏死的修复效果和远期疗效对比

Comparison of the Repair Effect and Long-term Effect of Modified Onlay and Full Crown Restoration in the Treatment of Pulp Necrosis

目的:比较改良式高嵌体与全冠修复治疗牙髓坏死的修复效果及远期疗效差异。方法:选取2015年6月-2018年1月在福建医科大学附属南平第一医院接受救治的牙髓坏死患者346例,随机数字表法分组。试验组(n=173)予以改良式高嵌体修复,对照组(n=173)予以全冠修复,比较两组患者不同时间段修复效果及牙齿健康情况。结果:修复后7 d,两组间修复效果优良率均为100%,差异无统计学意义(P>0.05);修复后3年,试验组修复效果优良率(97.11%)高于对照组(89.02%),差异有统计学意义(P<0.05)。修复后7 d及修复后3年,试验组PLI、GI评分均低于对照组,差异有统计学意义(P<0.05)。修复后3年内试验组牙齿松动(51.45%)、牙齿折断(5.20%)、牙齿拔除发生率(6.94%)均低于对照组,差异有统计学意义(P<0.05)。结论:改良式高嵌体与全冠修复治疗牙髓坏死均有良好修复效果,但改良式高嵌体远期疗效更好,患者牙齿更加健康且问题更少,值得推广。

Objective:To compare the repair effect and long-term effect of modified onlay and full crown restoration on pulp necrosis.Method:A total of 346 patients with pulp necrosis who treated in Nanping First Hospital of Fujian Medical University from June 2015 to January 2018 were selected as the research objects,and they were divided into two groups according to the random number table method.The experimental group(n=173)received modified onlay restoration,and the control group(n=173)received full crown restoration.The restoration effect and dental health of the two groups of patients were compared in different time periods.Result:After 7 days of restoration,the excellent and good rate of repair effect in two groups was 100%,the difference was not statistically significant(P>0.05).After 3 years of restoration,the excellent and good rate of repair effect in the experimental group(97.11%)was higher than that in the control group(89.02%),which was statistically significant(P<0.05).After 7 days and 3 years restoration,the PLI and GI scores of the experimental group were lower than those of the control group,which were statistically significant(P<0.05).Within 3 years after restoration,the incidence of tooth loosening(51.45%),tooth fracture(5.20%),and tooth extraction(6.94%)in the experimental group were lower than those in the control group,which were statistically significant(P<0.05).Conclusion:Modified onlay and full crown restorations have good repair effects in the treatment of pulp necrosis,but the modified onlay have better long-term effects,and patients have healthier teeth and fewer problems,which are worthy of promotion.
