Application Effect of Different Treatment Schemes in Patients with Incomplete Abortion

目的:对比不同治疗方案在不全流产患者中的应用效果。方法:选取2020年1月-2021年1月佛山市三水区人民医院就诊的120例不全流产患者作为研究对象。根据随机数字表法将其分为复方短效口服避孕药组、米非司酮组、黄体酮组及清宫组,每组30例。四组分别给予屈螺酮炔雌醇、米非司酮、黄体酮及清宫治疗。比较四组阴道流血时间、月经复潮天数,治疗后7 d血hCG下降值、子宫残留率、子宫内膜厚度,并发症发生率。结果:清宫组阴道流血时间短于其余三组(P<0.05),并发症发生率显著高于其余三组(P<0.05)。清宫组血hCG下降值低于其余三组,月经复潮天数短于其余三组,子宫内膜厚度、子宫残留率低于其余三组,但比较差异均无统计学意义(P>0.05)。复方短效口服避孕药组、米非司酮组、黄体酮组阴道流血时间及血hCG下降值,月经复潮天数及子宫内膜厚度,子宫残留率、并发症发生率比较差异均无统计学意义(P>0.05)。结论:复方短效口服避孕药、米非司酮、黄体酮、清宫术四种方案治疗不全流产均有一定的效果,其中,清宫术治疗效果最佳,但是,其会增加患者并发症发生率,其余三种方法效果相对较弱,但相对安全,患者可根据自身需求合理选择相应的方案治疗。

Objective:To compare the application effect of different treatment schemes in patients with incomplete abortion.Method:A total of 120 patients with incomplete abortion treated in Foshan Sanshui District People’s Hospital from January 2020 to January 2021 were selected as the research object.They were divided into compound short acting oral contraceptive group,Mifepristone group,Progesterone group and curettage group according to the random number table method,with 30 cases in each group.The four groups were treated with Drospirenone and Ethinylestradiol,Mifepristone,Progesterone and curettage respectively.The time of vaginal bleeding,the days of menstrual recovery,the decreased value of blood hCG,the residual rate of uterus,the thickness of endometrium 7 dafter treatment and the incidence of complications were compared in the four groups.Result:The vaginal bleeding time in the curettage group was shorter than in the remaining three groups(P<0.05),and the complication rate was significantly higher than in the remaining three groups(P<0.05).The blood hCG decline in the curettage group was lower than the remaining three groups,menstruation days were shorter than those in the remaining three groups,and endometrial thickness and uterine residual rate were lower than the remaining three groups,but the difference was not significant(P>0.05).There were no differences in vaginal bleeding time and blood hCG decrease,days of re-menstruation,endometrial thickness,uterine residual rate and complication rate in compound short acting oral contraceptive group,Mifepristone group,Progesterone group(P>0.05).Conclusion:Compound short acting oral contraceptive,Mifepristone,Progesterone,curettage has certain effects in the treatment of incomplete abortion.Among them,curettage has the best effect,but it will increase the incidence of complications of patients.The effect of the other three methods is relatively weak,but relatively safe.Patients can reasonably choose the corresponding treatment scheme according to their own needs.
