微血管减压术治疗面肌痉挛合并同侧三叉神经痛的临床研究

Clinical Study of Microvascular Decompression in the Treatment of Hemifacial Spasm with Ipsilateral Trigeminal Neuralgia

目的:探究微血管减压术对面肌痉挛合并同侧三叉神经痛的临床治疗效果及安全性。方法:选取南阳市第一人民医院神经外科2019年3月-2021年3月收治的124例面肌痉挛合并同侧三叉神经痛的患者为研究对象。采用随机分组法,分为A组和B组,每组62例。A组采用微血管减压术治疗,B组采用传统的面神经部分切断术治疗。比较两组临床疗效、术后并发症发生情况及生活质量的差异性。结果:A组临床总有效率(93.55%)高于B组(53.23%),差异有统计学意义(P<0.05)。A组并发症发生率(19.35%)低于B组(61.29%),差异有统计学意义(P<0.05)。手术前,两组生活质量评分比较,差异无统计学意义(P>0.05);术后3周,A组生活质量评分明显高于B组,差异有统计学意义(P<0.05)。结论:微血管减压术比传统性面神经切断术在治疗面肌痉挛合并同侧三叉神经痛中的效果显著。

Objective:To explore the clinical efficacy and safety of microvascular decompression for hemifacial spasm combined with ipsilateral trigeminal neuralgia.Method:A total of 124 patients with hemifacial spasm complicated with ipsilateral trigeminal neuralgia treated in the department of neurosurgery,Nanyang City First People’s Hospital from March 2019 to March 2021 were selected as the research objects.They were randomly divided into group A and group B,with 62 cases in each group.The group A was treated with microvascular decompression,and the group B was treated with traditional partial facial nerve amputation.The differences of clinical efficacy,postoperative complications and quality of life between the two groups were compared.Result:The total effective rate of clinical curative effect(93.55%)in the group A was higher than that in the group B(53.23%),and the difference was statistically significant(P<0.05).The complication rate of the group A(19.35%)was lower than that of the group B(61.29%),and the difference was statistically significant(P<0.05).Before surgery,there was no significant difference in quality of life scoring scores between the two groups(P>0.05).Three weeks after operation,the quality of life score in the group A was significantly higher than that in the group B,and the difference was statistically significant(P<0.05).Conclusion:Microvascular decompression is more effective than traditional facial neurotomy in the treatment of hemifacial spasm with ipsilateral trigeminal neuralgia.
