摘要
目的:探讨酸冰刺激疗法联合肌电生物反馈疗法(electro-myographic biofeedback therapy,EMGBFT)对脑卒中后吞咽障碍的影响。方法:选取咸宁市中医医院2018年3月-2020年3月收治的脑卒中后吞咽障碍84例患者作为研究对象。按照随机数字表法将其分为观察组和对照组,每组42例。对照组接受常规对症支持治疗,观察组在此基础上运用酸冰刺激疗法联合EMGBFT治疗。比较两组临床疗效、治疗前后舌骨上肌群肌电积分值(integrated electromyography,iEMG)、治疗前后营养状况[肱三头肌皮褶厚度(triceps skin-fold thickness,TSF)、上臂肌围(arm muscle circumference,AMC)、血清前白蛋白(prealbumin,PA)、白蛋白(albumin,ALB)]。结果:治疗后,观察组总有效率为95.24%,明显高于对照组的73.81%(P<0.05),临床疗效整体上优于对照组(P<0.05)。治疗后,观察组iEMG值明显高于对照组(P<0.05)。治疗后,观察组血清PA、ALB水平均显著高于对照组(P<0.05)。结论:酸冰刺激疗法联合EMGBFT治疗脑卒中后吞咽障碍有助于改善患者吞咽功能,保证营养摄入量,改善营养状况,提高康复疗效。
Objective:To explore effect of acid ice stimulation therapy combined with electro-myographic biofeedback therapy(EMGBFT)on dysphagia after stroke.Method:A total of 84 patients with dysphagia after stroke admitted to Xianning Hospital of Traditional Chinese Medicine from March 2018 to March 2020 were selected as the research objects.According to the random number table method,they were divided into observation group and control group,with 42 cases in each group.The control group received routine symptomatic support treatment,and the observation group was treated with acid ice stimulation therapy combined with EMGBFT on the above basis.The clinical efficacy,integrated electromyography(iEMG)of suprahyoid muscle group before and after treatment,nutritional status[triceps skin-fold thickness(TSF),arm muscle circumference(AMC),serum prealbumin(PA),albumin(ALB)]before and after treatment were compared between the two groups.Result:After treatment,the total effective rate of the observation group was 95.24%,which was significantly higher than 73.81%of the control group(P<0.05),and the whole clinical curative was better than that of the control group(P<0.05).After treatment,the iEMG value of the observation group was significantly higher than that of the control group(P<0.05).After treatment,the levels of serum PA and ALB in the observation group were significantly higher than those in the control group(P<0.05).Conclusion:Acid ice stimulation therapy combined with EMGBFT in the treatment of dysphagia after stroke is helpful to improve patients’swallowing function,ensure nutritional intake,improve nutritional status and improve rehabilitation curative effect.
