摘要
目的了解瑞安地区导致肝脓肿的肺炎克雷伯菌的药敏、毒力和分子流行病学特征。方法对本地区2018年6月—2021年6月分离自肝脓肿患者的66株肺炎克雷伯菌进行药敏测试、PCR检测K1、K2荚膜抗原和rmpA黏液表型调节基因,使用多位点序列分析进行全球数据比对。结果66株肺炎克雷伯菌药敏除氨苄西林外全部敏感;血清分型中K1、K2和非K1、K2型分别为28株、22株和16株;K1、K2型和非K1、K2血清型分别有96.4%、90.9%、18.8%rmpA基因阳性;MLST分型显示ST23有30株,ST444有7株,ST30有4株,ST1005、ST65各3株,ST25、ST86、ST217、ST218、ST375、ST382各2株,ST87、ST380、ST434、ST660、ST700、ST806、ST1049各1株。结论本地区导致肝脓肿的肺炎克雷伯菌对各种药物均比较敏感;主要是高毒力的K1和K2血清型;MLST分型以ST23为主。
Objective To investigate the antibiotic sensitivity,toxicity and molecular epidemiology ofKlebsiella pneumoniae causing liver abscess in Rui.an.Methods From June 2018 to June 2021,66 strains ofKlebsiella pneumoniaeisolated from patients with liver abscess in this region were tested for drug sensitivity,and PCR was used to detect K1 and K2 capsule antigens andrmpAmucous phenotypic regulation genes.Multi-locus sequence analysis was used to compare global data.Results All the 66 strains ofKlebsiella pneumoniaewere sensitive except ampicillin.The serotypes K1,K2 and non-K1,K2 were accounted for 28,22 and 16,respectively.There were 96.4%,90.9%and 18.8%positivermpAgene in K1,K2 and non-K1,K2serotypes,respectively.MLST typing showed that there were 30 strains of ST23,7 strains of ST444,4 strains of ST30,3 strains of ST1005 and ST65 respectively,and 2 strains of ST25,2 strains of ST86,2 strains of ST217,2 strains of ST218,2 strains of ST375 and 2 strains of ST382 respectively.1 strain of ST87,1 strain of ST380,1 strain of ST434,1 strain of ST660,1 strain of ST700,1 strain of ST806,1 strain of ST1049,respectively.Conclusion Klebsiella pneumoniaecausing liver abscess in Rui.an is sensitive to various drugs,mainly the highly virulent K1 and K2 serotypes,and ST23 is the dominant in MLST typing.
