摘要
目的探讨创伤性颅脑损伤患者血管内皮生长因子和脑源性神经营养因子水平及意义。方法回顾性分析2018年1月—2021年6月收治的98例创伤性颅脑损伤患者的临床资料,比较不同病情程度和不同预后患者的血清学指标水平,并分析血清学指标与GCS评分的关系以及血清学指标预测预后的价值。结果随着病情的加重,VEGF、S100β水平逐渐升高(P<0.05),BDNF水平逐渐下降,差异有统计学意义(P<0.05);创伤性颅脑损伤患者VEGF水平与GCS评分呈负相关(P<0.05),BDNF与GCS评分呈正相关(P<0.05),S100β与GCS评分无显著相关性(P>0.05);预后良好组患者VEGF水平显著低于预后不良组(P<0.05),BDNF水平显著高于预后不良组,差异有统计学意义(P<0.05);VEGF、BDNF以及二者联合检测预测预后的AUC分别为0.794、0.766、0.891,二者联合预测预后的AUC显著高于单一指标(P<0.05)。结论创伤性颅脑损伤患者VEGF、BDNF水平与病情程度和预后有关,可作为早期判断病情和预后的辅助指标。
Objective To investigate the levels and significance of vascular endothelial growth factor( VEGF) and brain-derivedneurotrophic factor( BDNF) in patients with traumatic brain injury. Methods Clinical data of 98 patients with traumatic brain injury admitted from January 2018 to June 2021 were retrospectively analyzed. The serological indexes of patients with different disease severity and prognosis were compared. The relationship between serological indexes and GCS score was analyzed,and the value of serological indexes in predicting poor prognosis was analyzed. Results With the aggravation of the disease,the levels of VEGF and S100β were gradually increased( P < 0. 05),while the level of BDNF was gradually decreased,with the differences of statistical significance( P < 0. 05). VEGF level was negatively correlated with GCS score( P < 0. 05),BDNF was positively correlated with GCS score( P < 0. 05),S100β was not significantly correlated with GCS score( P >0. 05). VEGF level in good prognosis group was significantly lower than that in poor prognosis group( P < 0. 05),BDNF level was significantly higher than that in poor prognosis group,and the differences were statistically significant( P < 0. 05). The AUC of VEGF,BDNF and their combination in predicting prognosis were 0. 794,0. 766 and 0. 891,respectively,and the AUC of the combination of VEGF and BDNF in predicting prognosis was significantly higher than that of the single indicator( P <0. 05). Conclusion The levels of VEGF and BDNF in patients with traumatic brain injury are related to the degree of disease and prognosis,and they can be used as auxiliary indicators for early assessment of the disease and prognosis.
