草酸艾司西酞普兰联合氨氯地平治疗中青年高血压伴抑郁症的临床效果研究

Clinical efficacy of escitalopram oxalate combined with amlodipine on young and middle-aged hypertension with depression

目的探讨草酸艾司西酞普兰联合氨氯地平治疗中青年高血压伴抑郁症的临床效果。方法选取2019年2月至2020年12月就诊于河北省人民医院心内科的高血压伴抑郁症患者100例。按照随机数字表法将入选者分成对照组(50例)及观察组(50例)。对照组单纯氨氯地平口服治疗,观察组在对照组基础上联合草酸艾司西酞普兰口服治疗。2组患者均治疗8周,比较2组治疗前后24 h平均收缩压(24 h mSBP)、24 h平均舒张压(24 h mDBP)、汉密尔顿抑郁量表(HAMD)评分、生活质量量表(QOL)评分。结果治疗前2组24 h mSBP、24 h mDBP水平及HAMD评分、QOL评分比较差异均无统计学意义(均P>0.05)。治疗后观察组24 h mSBP、24 h mDBP水平及HAMD评分均明显低于对照组[(125±12)mmHg(1 mmHg=0.133 kPa)比(132±13)mmHg、(90±3)mmHg比(92±3)mmHg、(9±5)分比(13±8)分],2组患者QOL评分明显高于治疗前,且观察组明显高于对照组[(9.9±1.5)分比(7.9±2.0)分],差异均有统计学意义(均P<0.05)。结论草酸艾司西酞普兰联合氨氯地平治疗中青年高血压伴抑郁症患者,可明显改善患者的血压水平且对改善患者抑郁心理有积极作用,并可有效提升患者的生活质量。

Objective To investigate the clinical efficacy of escitalopram oxalate combined with amlodipine on young and middle-aged adults with hypertension and depression.Methods Totally 100 patients with hypertension and depression admitted to Department of Cardiology,Hebei General Hospital from February 2019 to December 2020 were selected.Patients were divided into control group(50 cases)and observation group(50 cases)according to random number table method.The control group was treated with amlodipine orally,and the observation group was treated with escitalopram oxalate on the basis of the control group.Both groups were treated for 8 weeks.Levels of 24 h mean systolic blood pressure(24 h mSBP),24 h mean diastolic blood pressure(24 h mDBP),Hamilton Depression Scale(HAMD)and quality of life scale(QOL)were compared between the two groups before and after treatment.Results Before treatment,there were no significant differences in the levels of 24 h mSBP,24 h mDBP,HAMD score and QOL score between the two groups(all P>0.05).After treatment,the levels of 24 h mSBP,24 h mDBP and HAMD score in the observation group were significantly lower than those in the control group[(125±12)mmHg vs(132±13)mmHg,(90±3)mmHg vs(92±3)mmHg,(9±5)vs(13±8)];the QOL scores in the two groups were significantly higher than those before treatment,and the score in the observation group was significantly higher than that in the control group[(9.9±1.5)vs(7.9±2.0)](all P<0.05).Conclusion Escitalopram oxalate combined with amlodipine can significantly control blood pressure of young and middle-aged patients with hypertension and depression,play a positive role in improving the depressive psychology,and effectively improve the quality of life.
