慢性心力衰竭患者自我护理维持的现状及影响因素分析

Analysis of the current situation and influencing factor of self-care maintenance of patients with chronic heart failure

目的探讨慢性心力衰竭患者的自我护理维持的现状及影响因素。方法选取2019年2—9月于首都医科大学附属北京安贞医院住院治疗的慢性心力衰竭患者104例。使用中文版心力衰竭自我护理指数量表V6.2评估患者自我护理维持情况,最终获得有效问卷103份。采用多元线性回归模型分析影响慢性心力衰竭患者自我护理维持的影响因素。结果103例心力衰竭患者自我护理维持得分为(64±14)分,达标率为36.9%(38/103)。单因素分析结果显示,居住地、糖尿病及心力衰竭病程与患者自我护理维持得分相关(均P<0.05)。多元线性回归分析结果显示,糖尿病是影响心力衰竭患者自我护理维持的独立危险因素(标准回归系数=-0.216,P=0.024),居住地为城镇、心力衰竭病程>6年是独立保护因素(标准回归系数=0.190、0.286,P=0.047、0.024)。结论慢性心力衰竭患者的自我护理维持情况亟待改善。糖尿病是影响慢性心力衰竭患者自我护理维持的独立危险因素,居住地为城镇、心力衰竭病程>6年是独立保护因素。

Objective To explore the current situation and influencing factor of self-care maintenance of patients with chronic heart failure.Methods A total of 104 patients with chronic heart failure admitted to Beijing Anzhen Hospital,Capital Medical University from February to September 2019 were enrolled.The Chinese heart failure self-care index scale V6.2 was used to assess the situation of self-care maintenance in patients with chronic heart failure,and 103 valid questionnaires were obtained finally.Multiple linear regression model was used to analyze the influencing factors for self-care maintenance in patients with chronic heart failure.Results The score of self-care maintenance of patients with chronic heart failure was(64±14)and the pass rate was 36.9%(38/103).Univariate analysis showed that residence,diabetes mellitus and course of heart failure were correlated with the score of self-care maintenance(all P<0.05).Multiple linear regression analysis showed that diabetes mellitus was independent risk factor for self-care maintenance(standard regression coefficient=-0.216,P=0.024),and living in town and course of heart failure>6 years were independent protective factors for that(standard regression coefficient=0.190,0.286;P=0.047,0.024).Conclusion The self-care maintenance of patients with chronic heart failure needs to be improved.Diabetes mellitus is independent risk factor for self-care maintenance of patients with chronic heart failure,and living in town and course of heart failure>6 years were independent protective factors for that.
