肩关节镜下单排间断缝合与双排缝合治疗肩袖损伤的临床对比研究

Clinical comparative study of single row interrupted suture and double row suture in the treatment of rotator cuff injury under arthroscopy

目的对比分析肩关节镜下单排间断缝合与双排缝合治疗肩袖损伤的临床效果,为临床医师选择合理术式提供参考。方法选取2017年1月—2020年12月期间康平县人民医院诊治的274例肩袖损伤患者作为研究对象,依据随机数字表法分为对照组和观察组各137例。2组均采用肩关节镜下微创治疗,其中对照组行单排间断缝合,观察组行双排缝合,对比2组患者术后3个月的简明肩关节功能测试(SST)评分、欧洲肩关节协会制定的Constant肩关节评分系统得分以及肩关节活动度。结果观察组患者术后3个月的SST评分高于对照组(P<0.05),Constant肩关节评分系统中疼痛、日常活动、肩关节活动范围、力量四项评分均高于对照组(P<0.05),肩关节活动度中前屈、后伸、外展、内收、上举度均高于对照组(P<0.05)。结论在肩关节镜下微创治疗肩袖损伤时采用双排缝合的临床效果更佳,可有效改善患者肩关节功能,提高肩关节活动度,值得推广。

Objective To compare and analyze the clinical effect of single row interrupted suture and double row suture in the treatment of rotator cuff injury under arthroscopy,so as to provide reference for clinicians to choose reasonable surgical methods.Methods A total of 274 patients with rotator cuff injury diagnosed and treated in Kangping People's Hospital from January 1,2017 to December 31,2020 were selected as the research objects.According to the random number table method,the research subjects were divided into two groups:the control group and the observation group(137 cases in each group).Both groups were treated with minimally invasive treatment under shoulder arthroscopy,among them,the control group was treated with single row interrupted suture,and the observation group was treated with double row suture.The simple shoulder function test(SST)score,the constant shoulder score system developed by European shoulder Association and the range of motion of the shoulder at three months after operation were compared between the two groups.Results At three months after operation,the SST score of the observation group was higher than that of the control group(P<0.05).Three months after operation,the pain,daily activities,shoulder range of motion and strength scores of the observation group were higher than those of the control group(P<0.05).Three months after operation,the range of motion of shoulder joint in the observation group was higher than that in the control group(P<0.05).Conclusion When the rotator cuff injury was treated with minimally invasive surgery under arthroscopy,the clinical effect of double row suture is better,which can effectively improve the function of shoulder joint and the range of motion of shoulder joint,which is worthy of promotion.
