目的探究胃癌早期筛查中应用糖类抗原199(carbohydrate antigen199,CA199)、糖类抗原125(carbohydrate antigen125,CA125)、甲胎蛋白(alpha fetoprotein,AFP)、癌胚抗原(carcinoembryonic antigen,CEA)及糖类抗原153(carbohydrate antigen153,CA153)联合检测的价值。方法选择2019年12月—2021年10月龙南市第一人民医院接收的胃癌早期患者40例作为研究对象(观察组),同期体检的健康人士40例作为参照对象(对照组),均行CA199、CA125、AFP、CEA、CA153检测。比较2组CA199、CA125、AFP、CEA、CA153水平差异,并分析CA199、CA125、AFP、CEA、CA153单一检测与联合检测的灵敏度、特异度、准确度、阳性预测值、阴性预测值。结果观察组患者CA199、CA125、AFP、CEA、CA153水平均高于对照组,差异有统计学意义(P<0.05)。联合检测灵敏度、特异度、准确度、阳性预测值、阴性预测值均高于单一检测,差异有统计学意义(P<0.05)。结论在胃癌早期筛查中应用CA199、CA125、AFP、CEA、CA153联合检测的效果显著,诊断准确性高,能够为临床诊治提供科学依据,值得推广。

Objective To explore the value of combined detection of carbohydrate antigen 199(CA199),carbohydrate antigen 125(CA125),alpha fetoprotein(AFP),carcinoembryonic antigen(CEA)and carbohydrate antigen 153(CA153)in early gastric cancer screening.Methods 40 patients with early gastric cancer in our hospital from December 2019 to October 2021 were selected as the research object(observation group),and 40 healthy people who came to our hospital for physical examination in the same period were selected as the reference object(control group).CA199,CA125,AFP,CEA and CA153 were detected.The levels of CA199,CA125,AFP,CEA and CA153 were compared between the two groups,and the sensitivity,specificity,accuracy,positive predictive value and negative predictive value of single and combined detection of CA199,CA125,AFP,CEA and CA153 were analyzed.Results the levels of CA199,CA125,AFP,CEA and CA153 in patients in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).The sensitivity,specificity,accuracy,positive predictive value,and negative predictive value of the combined test were higher than those of single tests,and the differences were statistically significant(P<0.05).Conclusion The combined detection of CA199,CA125,AFP,CEA and CA153 in early gastric cancer screening has significant effect,high disease diagnosis rate and diagnostic accuracy.It can provide scientific basis for clinical diagnosis and treatment,and is worthy of popularization.
