高强度聚焦超声在I型子宫瘢痕妊娠治疗中的应用价值

Clinical application of high-intensity focused ultrasound in type I cesarean scar pregnancy

目的探讨高强度聚焦超声在子宫瘢痕妊娠治疗中的应用价值。方法通过前瞻性队列研究将东莞市妇幼保健院2019年1月—12月收治的239例Ⅰ型子宫瘢痕妊娠患者,根据住院号奇偶性随机分为试验组117例和对照组122例。试验组接受高强度聚焦超声作为术前预处理,后联合宫腔镜下清宫术;对照组接受单纯宫腔镜下清宫术,对比2组治疗效果。结果试验组的成功率(97.4%)明显高于对照组(90.2%)(P<0.05),术中出血量、宫腔内包块消失时间以及术后并发症发生率均显著优于对照组(P<0.05)。结论高强度聚焦超声联合宫腔镜下清宫术治疗Ⅰ型子宫瘢痕妊娠临床疗效更好,安全性更高。

Objective To investigate the safety and efficacy of high intensity focused ultrasound in the treatment of cesarean scar pregnancy.Methods 239 patients with type I cesarean scar pregnancy admitted to our hospital from January to December 2019 were randomly divided into experimental group(n=117)and control group(n=122)by prospective cohort study.The HIFU group received high-intensity focused ultrasound as preoperative management,followed by hysteroscopic-uterine curettage;the control group received hysteroscopic uterine curettage without any pretreatment,and the therapeutic effects were compared between the two groups.Results The success rate of the experimental group was significantly higher than that of the control group(P<0.05),the intraoperative blood loss,the disappearance time of intrauterine mass and the incidence of postoperative complications in the experimental group were significantly better than those in the control group(P<0.05).Conclusion High-intensity focused ultrasound combined with hysteroscopic uterine curettage in the treatment of type I cesarean scar pregnancy is a safe and effective modality.
