摘要
目的:研究人免疫球蛋白在小儿肺炎中临床应用及对患儿T细胞水平的影响。方法:选取安徽省儿童医院100例肺炎患儿,随机数表法分为观察组与对照组,两组各50例。对照组予常规治疗,观察组另静注人免疫球蛋白。比较两组患者的治疗效果、临床症状改善(止咳、啰音消失、体温正常、呼吸困难缓解)时间、治疗前后T淋巴细胞(CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+))水平及治疗期间不良反应发生率。结果:观察组总有效率88.00%显著高于对照组70.00%;观察组止咳、啰音消失、体温正常、呼吸困难缓解显著少于对照组;观察组治疗后CD4^(+)、CD4^(+)/CD8^(+)高于治疗前及对照组,CD8^(+)低于治疗前及对照组;两组治疗期间不良反应总发生率分别为10.00%、4.00%,比较差异无统计学意义。结论:人免疫球蛋白能提升小儿肺炎患儿CD4^(+)、CD4^(+)/CD8^(+)水平,降低CD8^(+)水平,同时应用于疾病的治疗可获得较好的疗效。
Objective To study the clinical application of human immunoglobulin in children with pneumonia and its effect on the level of T cells in children.Methods A total of 100 children with pneumonia in Anhui Children’s Hospital were randomly divided into observation group and control group,with 50 cases in each group.The control group received routine treatment,and the observation group received intravenous injection of human immunoglobulin.The treatment effect,time to improve clinical symptoms (cough,rale disappearance,normal temperature,relief of dyspnea),T lymphocyte (CD4^(+),CD8^(+),CD4^(+)/CD8^(+)) levels before and after treatment and the incidence of adverse reactions during treatment were compared between the two groups.Results The total effective rate of 88.00% in the observation group was significantly higher than 70.00% in the control group.The cough relief,rale disappearance,normal body temperature,relief of dyspnea in the observation group were significantly less than those in the control group.After treatment,CD4^(+)and CD4^(+)/CD8^(+)in observation group were higher than before treatment and control group,while CD8^(+)was lower than before treatment and control group.The total incidence of adverse reactions in the two groups were 10.00% and 4.00% respectively.There was no significant difference between the two groups.Conclusion Human immunoglobulin can increase the levels of CD4^(+),CD4^(+)/CD8^(+)and reduce the level of CD8^(+)in children with pneumonia.At the same time,it can be used in the treatment of diseases.
