血清尿酸/肌酐比值与骨质疏松的相关性研究 被引量:3

摘要 目的 探讨血清尿酸/肌酐比值(UA/Cr)与骨质疏松的关系。方法 选取本院骨质疏松门诊就诊者282例为研究对象,根据骨密度检查结果将所有研究对象分为骨质疏松组(n=154)和正常对照组(n=128),测定所有研究对象的血尿酸和血肌酐水平,计算UA/Cr。结果 骨质疏松组UA/Cr显著低于正常对照组(P <0.01);Logistic回归分析提示,校正一些危险因素如年龄、性别、血脂、血糖后,UA/Cr是骨质疏松发生的独立保护因素(P <0.05)。随着UA/Cr水平的升高,骨质疏松发生率逐渐降低(75.53%vs 54.26%vs 34.04%,P <0.05)。ROC曲线分析提示UA/Cr对骨质疏松的发生有较好的预测价值,曲线下面积为0.793(95%CI:0.742~0.845,P <0.001),优于血清UA对骨质疏松的预测价值。结论 UA/Cr是骨质疏松发生的独立保护因素,可作为预测骨质疏松发生的一种可靠的血清学标记物。 Objective To investigate the relationship between serum uric acid/creatinine ratio(UA/Cr)and osteoporosis.Methods A total of 282 participants from osteoporosis clinic in our hospital were enrolled and they were divided into the osteoporosis group(n=154)and the control group(n=128)according to the results of bone mineral density examination.Serum uric acid and creatinine levels of all subjects were determined,and UA/Cr was calculated.Results The UA/Cr levels in the osteoporosis group were significantly lower than those in the control group(P<0.01).Logistic regression analysis showed that UA/Cr was an independent protective factor of osteoporosis after adjusting risk factors such as age,"sex,blood lipid and blood glucose(P<0.05).With the increase of UA/Cr levels,the incidence of osteoporosis decreased gradually(75.53%vs 54.26%vs 34.04%,P<0.05).ROC curve analysis showed that UA/Cr had a good predictive value for the occurrence of osteoporosis.The area under the curve(AUC)was 0.793(95%CI:0.742-0.845,P<0.001),which was better than the predictive value of serum UA for osteoporosis.Conclusion UA/Cr is an independent protective factor for osteoporosis,and should be used as a reliable serological marker to predict the occurrence of osteoporosis.
