腹腔镜肠粘连松解术治疗粘连性肠梗阻20例临床分析

Laparoscopy for the treatment of intestinal adhesion release adhesion sex intestinal obstruction of 20 cases of clinical analysis

目的观察腹腔镜肠粘连松解术治疗粘连性肠梗阻的临床效果。方法选取2018年1月-2020年12月间北京中医医院平谷医院收治的粘连性肠梗阻患者40例,随机分成观察组与对照组各20例,对照组采取开腹手术治疗,观察组采取腹腔镜肠粘连松解术治疗,对2组手术指标、恢复情况、炎症因子水平、并发症等进行对比分析。结果与对照组患者相比,观察组手术操作时间更短、术中出血更少、术后恢复进食时间和腹痛消失时间更短、住院天数更少,差异均有统计学意义(P<0.05)。术前、术后5 d 2组患者PCT、CRP、TNF-α、IL-6水平对比无明显差异(P>0.05);术后1 d,观察组PCT、CRP、TNF-α、IL-6水平显著低于对照组(P<0.05)。观察组并发症总发生率显著低于对照组(P<0.05)。结论腹腔镜肠粘连松解术治疗粘连性肠梗阻临床效果明显优于开腹手术,值得推广。

Objective To observe and analyze the application effect of laparoscopic intestinal adhesion release in the treatment of adhesive ileus.Methods Between January 2018 and December 2020,adhesion sex intestinal obstruction patients of our hospital between 40,randomized divided into observation group and control group,20 cases in the control group treated with open operation,observation group adopt laparoscopic treatment of intestinal adhesion release,on two groups of operation index,recovery index and inflammatory factor levels and complications were analyzed.Results Compared with the control group,the observation group had shorter operation time,less intraoperative bleeding,shorter postoperative recovery time of eating and abdominal pain,and fewer days of hospitalization,with statistical significance(P<0.05).There were no significant differences in PCT,CRP,TNF-α and IL-6 levels between 2 groups before and 5 days after surgery(P>0.05).One day after operation,the levels of PCT,CRP,TNF-α and IL-6 in observation group were significantly lower than those in control group(P<0.05).The total incidence of complications in observation group was significantly lower than that in control group(P<0.05).Conclusion Laparoscopic intestinal adhesion release is superior to laparotomy in the treatment of intestinal adhesion obstruction,which is worth promoting.
