联合用药对特发性肺纤维化的疗效观察

Efficacy of drug combination in the treatment of idiopathic pulmonary fibrosis

目的探究特发性肺纤维化(IPF)患者应用汉防己甲素片对肺功能、肺纤维化指标及血清因子水平的影响。方法选取我院收治的IPF患者81例,采用抽签法将其分组。对照组40例给予吡非尼酮胶囊,观察组41例在对照组的基础上给予汉防己甲素片。治疗3个月后,对比两组患者临床疗效、肺功能指标、肺纤维化指标、血清因子水平。结果治疗后,观察组总有效率高于对照组(P<0.05);肺活量、最大呼气峰流速、第1秒用力呼气容积水平均高于对照组(P<0.05);观察组透明质酸、Ⅲ型胶原、Ⅳ型胶原水平均低于对照组(P<0.05);观察组胰岛素生长因子-I、血管内皮生长因子、血清转化生长因子-β1水平均低于对照组(P<0.05)。结论汉防己甲素联合吡非尼酮能够有效提高IPF患者治疗效果,提高肺功能,减轻肺纤维化程度和下调血清因子水平。

Objective To investigate the effect of tetrandrine tablets on pulmonary function,pulmonary fibrosis indexes and serum factor levels of patients with idiopathic pulmonary fibrosis(IPF).Methods 81 patients with IPF in our hospital were selected and randomly divided into the observation group and the control group.40 cases in the control group were given pirfenidone capsule,41 cases in the observation group were combined with tetrandrine tablets on the basis of the control group.After 3 months’treatment,the clinical efficacy,pulmonary function indicators,pulmonary fibrosis indicators and serum factor levels were compared between the two groups.Results After treatment,the total effective rate in observation group was higher than that in control group(P<0.05).The forced vital capacity(FVC),maximum peak expiratory flow rate(PEF)and forced expiratory volume in the first second(FEV1)in the observation group were higher than those in the control group(P<0.05).The levels of hyaluronic acid(HA),typeⅢcollagen(Ⅲ-c)and typeⅣcollagen(Ⅳ-C)in the observation group were lower than those in the control group(P<0.05).The levels of insulin growth factor-I(IGF-I),vascular endothelial growth factor(VEGF)and serum transforming growth factor-β1(TGF-β1)in observation group were lower than those in control group(P<0.05).Conclusion Tetrandrine combined with pirfenidone can effectively enhance the therapeutic effect of IPF patients,which can improve lung function,reduce the degree of pulmonary fibrosis,reduce the level of serum factors.
