右美托咪定复合罗哌卡因腹横肌平面阻滞用于妇科腹腔镜术后镇痛的效果观察

Observation on the Effect of Dexmedetomidine Combined with Ropivacaine for Transversus Abdominis Plane Block in Postoperative Analgesia of Gynecological Laparoscopy Surgery

目的 观察右美托咪定复合罗哌卡因腹横肌平面阻滞(TAPB)用于妇科腹腔镜术后镇痛的效果。方法 74例行妇科腹腔镜手术的患者,随机分为两组。手术结束后,两组均在超声引导下行双侧TAPB,对照组使用罗哌卡因镇痛,研究组使用右美托咪定复合罗哌卡因镇痛。比较两组的镇痛效果、麻醉恢复情况及不良反应。结果 术后4 h,两组的VAS评分比较,差异无统计学意义(P>0.05);术后8 h、 12 h、 24 h,研究组的VAS评分均显著低于对照组(P<0.05)。研究组的苏醒时间、自主呼吸恢复时间及定向力恢复时间均显著短于对照组(P<0.05)。两组的不良反应发生率比较,差异无统计学意义(P>0.05)。结论 右美托咪定复合罗哌卡因TAPB用于妇科腹腔镜术后镇痛效果显著,安全性高。

Objective To observe the effect of dexmedetomidine combined with ropivacaine for transversus abdominis plane block(TAPB) in postoperative analgesia of gynecological laparoscopy surgery. Methods 74 patients with gynecological laparoscopy surgery were randomly divided into two groups. After surgery, both groups received bilateral TAPB guided by ultrasound. The control group was given ropivacaine for analgesia, and the study group was given dexmedetomidine combined with ropivacaine for analgesia. The analgesic effect,anesthesia recovery and adverse reactions were compared between the two groups. Results At 4 h after surgery, no significant difference was found in the VAS score between the two groups(P >0.05);At 8, 12, and 24 h after surgery, the VAS scores of the study group were significantly lower than those of the control group(P <0.05). The awakening time, spontaneous breathing recovery time and orientation recovery time of the study group were significantly shorter than those of the control group(P <0.05). No significant difference was found in the incidence of adverse reactions between the two groups(P >0.05). Conclusions Dexmedetomidine combined with ropivacaine for TAPB in postoperative analgesia of gynecological laparoscopy surgery has significant effect and high safety.
