摘要
目的通过分析接受初次全膝关节置换术的患者,验证国人鞋子尺寸及膝关节假体大小的相关性及临床意义。方法对2018年12月至2021年10月安徽医科大学第一附属医院接受初次全膝关节置换的中国患者121例(132膝)进行单中心回顾性队列研究,男性34例(40膝),女性87例(92膝);年龄47~83岁,平均(66.03±7.49)岁;身高142~180 cm,平均(161.50±7.68)cm;体重46~95 kg,平均(66.64±10.16)kg;身体质量指数(body mass index,BMI)19.15~33.32 kg/m^(2),平均(25.55±3.38)kg/m^(2)。将患者的年龄、性别、身高、体重、假体厂家、胫骨假体型号、股骨假体型号采集记录,3种厂家的假体纳入研究。电话询问患者鞋子尺码并记录,对采集的数据进行Spearman’s秩相关性分析。结果Spearman’s秩相关性分析结果为鞋子尺码与胫骨假体前后径(anterior posterior,AP)(r=0.608,P<0.05),鞋子尺码与胫骨假体内外侧径(medial later,ML)(r=0.631,P<0.05),鞋子尺码与股骨假体AP(r=0.739,P<0.05),鞋子尺码与股骨假体ML(r=0.712,P<0.05),差异均具有统计学意义,均为强正相关。身高与胫骨假体AP(r=0.407,P<0.05),身高与胫骨假体ML(r=0.446,P<0.05),身高与股骨假体AP(r=0.536,P<0.05),身高与股骨假体ML(r=0.565,P<0.05),差异均具有统计学意义,均为中等正相关。结论国人鞋子尺码大小与初次全膝关节置换术假体大小存在正相关,此相关性可以提高骨科医生在术前规划中预估假体大小的准确性,经济有效。
Objective To verify the correlation and clinic significance of shoe size of Chinese and total knee arthroplasty(TKA)component size through analyzing patients undergoing primary TKA.Methods A single center retrospective cohort study was conducted on 121(132 knees)Chinese patients undergoing primary TKA from December 2018 to October 2021,including 34(40 knees)male patients and 87(92 knees)female patients.The range of age was 47 to 83 years,and the mean age was(66.03±7.49)years.The range of height was 142~180 cm,and the mean height was(161.50±7.68)cm.The range of weight was 46~95 kg,and the mean weight was(66.64±10.16)kg.The range of body mass index(BMI)was 19.15~33.32 kg/m^(2), and the mean BMI was(25.55±3.38)kg/m^(2).All data of patients including age,gender,height,weight,manufacturers of components,and size of tibial components and femoral components were collected and recorded,and three manufacturers were included in the study.Size of patient shoes was recorded by telephone interview.The spearman’s rank correlation was utilized to analyze the collected data.Results The statistically significant and good positive correlation outcomes of the spearman’s rank correlation analysis were verified as follows:shoe size and tibial component anterior posterior(AP)diameter(r=0.608,P<0.05);shoe size and tibial component medial lateral(ML)diameter(r=0.631,P<0.05);shoe size and femoral component AP diameter(r=0.739,P<0.05);shoe size and femoral component ML diameter(r=0.712,P<0.05).The statistically significant and moderate positive correlation outcomes were verified as follows:height size and tibial component AP diameter(r=0.407,P<0.05);height size and tibial component ML diameter(r=0.446,P<0.05);height size and femoral component AP diameter(r=0.536,P<0.05);height size and femoral component ML diameter(r=0.565,P<0.05).Conclusion There exists positive correlation of shoe size of Chinese and primary TKA component size.The correlation can improve the accuracy of estimating size of the prothesis in the preoperative planning for orthopedists,which is cost-effective.
