耳穴压豆及艾灸联合康复运动在老年患者全膝关节置换术后康复中的应用

Application of auricular acupoint pressing and moxibustion combined with rehabilitation exercise in elderly patients after total knee arthroplasty

目的 了解耳穴压豆及艾灸联合康复运动对全膝关节置换术后老年患者疼痛程度及关节功能恢复的影响。方法选择2019年1月至2021年1月在本科室住院行全膝关节置换术的膝骨性关节炎老年患者112例,按入院先后次序编号,采用随机数字表法,将112例患者随机分为对照组和试验组,每组各56例。对照组在术后进行常规康复功能锻炼,试验组在此基础上予艾灸、耳穴压豆疗法。干预前和干预后3d评估两组患者的疼痛程度;干预前和干预后7d评估两组患者膝关节功能和下肢肿胀程度。结果 干预后试验组患者疼痛评分(1.76±0.52)分低于对照组(4.23±0.65)分及疼痛程度低于对照组,差异有统计学意义(P<0.001,P<0.05)。干预后试验组患者膝关节功能得分(76.72±8.01)分高于对照组(66.52±5.81)分及下肢肿胀程度低于对照组,差异有统计学意义(P<0.01)。结论 耳穴压豆及艾灸联合康复运动不仅可减轻老年全膝关节置换术后患者疼痛程度和肢体肿胀程度,并且促进了患者膝关节功能的恢复。

Objective To investigate the effect of auricular acupoint bean pressing together with moxibustion for pain management in the rehabilitative exercise of elderly patient safter the surgery of total knee replacement. Methods One hundred and twelve patients who had total knee replacement surgery in our department, from January 2019 to January 2021, were enrolled and divided into acontrol group and an experimental group, with 56 patients per group. The former was offered with a routine rehabilitative exercise after the surgery, and the experimental group was given a treatment of moxibustion combined with an auricular acupoint bean pressing. The level of pain in both groups was assessed before the intervention and at 3 days after intervention. The knee function and lower limb swelling degree in both groups were evaluated before the intervention and 7d after the intervention. Results After the intervention, the score of pain in the experimental group was significantly lower than that of the control group(4.230.65 vs. 1.760.52, P <0.05) and the level of pain was significantly lower than that of the control group(P<0.05). After the intervention, the score of knee function in the experimental group was significantly higher than that of control group(76.728.01 vs. 66.525.81, P<0.01) and the lower limb swelling was significantly less than that of control group(P<0.01). Conclusions The auricular acupoint bean pressing combined with a moxibustionin the rehabilitation exercise can not only reduce the pain and swelling of lower limbs, but also promote the rehabilitation of knee function of the elderly patients after the total knee replacement surgery.
