粪菌移植治疗肠易激综合征

The treatment of irritable bowel syndrome by fecal bacteria transplantation

粪菌移植作为治疗肠易激综合征的一种新型治疗方法,通过改善受菌者内脏敏感性、肠道免疫以及肠道动力等多途径的机制调解肠道菌群、改善患者症状,日益受到国内外医学界的关注和认可。本文从粪菌移植的现代医学认识、肠易激综合征与肠道菌群的关系、治疗机制等方面回顾性地分析并综述了近年来粪菌移植治疗肠易激综合征的研究进展。作为重建肠道菌群的有效手段,粪菌移植被认为是近年来医学上的一个突破性进展,但目前仍受供体与受体关系、性别、年龄、心理及给药途径等多方面的制约,笔者认为应进一步通过大样本量的临床试验探索不同IBS亚型肠道微生物群的特异性、微生物群-宿主相互作用的潜在机制以及微生物群在疾病发病机制中的作用,以期建立粪便库,开展高效、精确、个体化的FMT治疗方案。

Fecal bacteria transplantation,as a new treatment method for irritable bowel syndrome,has been increasingly concerned and recognized by the medical community at home and abroad by improving the intestinal sensitivity,intestinal immunity,intestinal motility and other mechanisms to mediate intestinal flora and improve patients′symptoms.This paper reviews the research progress of fecal bacteria transplantation in the treatment of irritable bowel syndrome in recent years from the aspects of modern medical understanding,the relationship between irritable bowel syndrome and intestinal flora,and the therapeutic mechanism.As an effective way to reconstruct intestinal flora,fecal bacteria transplantation is considered as a breakthrough in medicine in recent years,but it is still restricted by the relationship between donor and recipient,gender,age,psychology and route of administration.We believe that further clinical trials with large sample sizes should be conducted to explore the specificity of intestinal microbiota of different IBS subtypes,the potential mechanism of microbiota-host interaction and the role of microbiota in the pathogenesis of the disease,in order to establish a stool bank to carry out efficient,accurate and individualized FMT treatment.
