胃间质瘤超声双重造影的时间-强度曲线与病理危险度分级的相关性研究

The correlation of the time-intensity curve of double contrast-enhanced ultrasonography and pathological risk classification of gastric stromal tumors

目的:探讨超声双重造影(DCEUS)的时间-强度曲线(TIC)与胃间质瘤(GSTs)危险度分级的相关性。方法:分析在我院住院治疗并经胃肠外科手术切除或胃镜下剥离的术后病理免疫组化证实的高危、低危GSTs患者共62例,分析DCEUS下TIC与其危险度分级的相关性。结果:低危GSTs DCEUS表现以环状增强、均匀低增强为主。高危GSTs DCEUS表现以环状增强、不均匀高增强为主。TIC下,低危GSTs造影剂AT、TTP、BI、PI分别为(10.71±2.05)s、(24.70±4.29)s、(23.80±4.07)dB、(52.61±1.28)d B,高危GSTs造影剂AT、TTP、BI、PI分别为(9.60±2.50)s、(24.17±4.23)s、(24.02±3.38)dB、(58.44±1.01)dB。低危与高危GSTs在PI比较,差异有统计学意义(Ρ<0.05)。绘制ROC曲线可知,当PI≥54.875 dB时,对GSTs危险度分级的正确率为80.65%(50/62),其中高危GSTs分级正确率为86.49%(32/37),低危GSTs分级正确率为72%(18/25)。结论:DCEUS的TIC对GSTs的危险度分级具有可参考的价值。

Objective:To evaluate the correlation of the time-intensity curve(TIC) of double contrast-enhanced ultrasonography and the risk classification of gastric stromal tumors.Methods:A total of 62 patients with high-risk and lowrisk GSTs confirmed by postoperative pathological immunohistochemistry after gastrointestinal surgery or gastrointestinal dissection were analyzed,and the correlation between the TIC under DCEUS with the risk grade of GSTs was analyzed.Results:Low-risk GSTs DCEUS performance is mainly characterized by circular enhancement and uniform low enhancement.High-risk GSTs DCEUS performance is mainly circular and uneven high enhancement.Under the TIC,the low-risk GSTs contrast agent AT,TTP,BI,PI is(10.71 ±2.05)s,(24.70 ±4.29)s,(23.80 ±4.07)dB,(52.61 ±1.28)dB,the high-risk GSTs contrast agent AT,TTP,BI,PI is(9.60±2.50)s,(24.17±4.23)s,(24.02±3.38)dB,(58.44±1.01)dB,in PI,with statistically significant(Ρ<0.05).For the ROC curve,when the PI ≥54.875 dB the accuracy of GSTs rating was 80.65%(50/62),where the high-risk GSTs rating was 86.49%(32/37),the low-risk GSTs rating was 72%(18/25).Conclusion:The TIC of DCEUS has referable value for the risk classification of GSTs.
