摘要
目的:利用干细胞特征相关自身抗体(AAbs)对早期非小细胞肺癌(NSCLC)患者进行初步检测,评估其早期诊断和精确预测预后的潜在价值。方法:采用酶联免疫吸附法(ELISA)测定血清中选择性抗体的浓度。共入组458例(训练集=401;验证集=57)。采用TCGA数据库分析相关基因的不同表达及预后价值。p53的最优分割值为11.60 U/mL,MAGEA1为4.90 U/mL,SOX2为3.85 U/mL,PGP9.5为7.05 U/mL。结果:干细胞特征相关抗体MAGEA1、PGP9.5、SOX2和TP53在非小细胞肺癌中高表达,与总生存期(OS)呈负相关(P<0.05)。在验证集中,四个候选小组的敏感性为66.7%,特异性为88.9%。四种抗体的检测在非小细胞肺癌的早期诊断中也表现出高特异性和敏感性,这进一步证明了其潜在的应用价值。结论:干细胞特征相关自身抗体可以用于早期非小细胞肺癌的早期诊断,但不能用于肿瘤的局部筛查,且其异常表达与患者较差的生存期一致。
Objective:To evaluate the potential value of early diagnosis and accurate prognosis of patients with early non-small cell lung cancer(NSCLC)by using stem cell characteristic associated self-antibodies(AAbs).Methods:The concentrations of selective antibodies in serum were quantitated by enzyme-linked immunosorbent assay(ELISA),and a total of 458 cases were enrolled for analysis(training set=401,validation set=57).TCGA databases was used to analyze the different expression and prognostic value of related genes.The optimal segmentation value of p53 was 11.60 U/mL,MAGEA1 was 4.90 U/mL,SOX2 was 3.85 U/mL and PGP9.5 was 7.05 U/mL.Results:The stem cell characteristic related antibodies MAGEA1,PGP9.5,SOX2 and TP53 were highly expressed in NSCLC and negatively correlated with overall survival(OS)(P<0.05).In the validation set,the sensitivity of the four candidate groups was 66.7%and the specificity was 88.9%.the detection of four antibodies also showed high specificity and sensitivity in the early diagnosis of NSCLC,which further proved its potential application value.Conclusion:The stem cell characteristic associated self-antibodies could be used as effective indicators in the early diagnosis of early NSCLC,but they can not be applied for local tumor screening,and their abnormal expression is consistent with the poor survival time of patients.
