奥氮平联合重复经颅磁刺激治疗对精神分裂症患者症状及认知的影响 被引量:1

摘要 目的 探讨奥氮平联合重复经颅磁刺激治疗对精神分裂症患者症状及认知的影响。方法 50例精神分裂症患者,随机分为对照组和观察组,每组25例。对照组患者口服奥氮平治疗,观察组患者采用奥氮平联合重复经颅磁刺激治疗。对比两组治疗前后阳性和阴性精神症状评定量表(PANSS)评分及精神分裂症认知功能成套测验共识版(MCCB)评分。结果 治疗前,两组的PANSS阳性症状评分、一般精神病理评分、阴性症状评分、总分比较差异无统计学意义(P>0.05);治疗后,两组的PANSS阳性症状评分、一般精神病理评分、阴性症状评分、总分均明显低于治疗前,且观察组的PANSS一般精神病理评分(30.12±2.78)分、阴性症状评分(16.24±1.48)分、总分(63.82±10.15)分均明显低于对照组的(35.29±4.15)、(19.13±2.75)、(71.55±10.53)分,差异具有统计学意义(P<0.05)。两组治疗后的PANSS阳性症状评分比较差异无统计学意义(P>0.05)。治疗前,两组的处理速度总分、连线测验A评分、动物命名评分和符号编码评分比较差异均无统计学意义(P>0.05);治疗后,两组的处理速度总分、连线测验A评分、动物命名评分和符号编码评分均明显高于本组治疗前,且观察组的处理速度总分(32.27±2.34)分、连线测验A评分(34.57±4.38)分、动物命名评分(38.47±4.27)分和符号编码评分(26.45±5.13)分均明显高于对照组的(30.59±2.28)、(31.26±5.79)、(34.29±5.24)、(21.39±5.34)分,差异具有统计学意义(P<0.05)。结论 奥氮平联合重复经颅磁刺激治疗可以明显减轻精神分裂症患者的症状,改善其认知功能。 Objective To discuss the effect of olanzapine combined with repetitive transcranial magnetic stimulation on symptoms and cognition of patients with schizophrenia.Methods A total of 50 patients with schizophrenia were randomly divided into control group and observation group,with 25 cases in each group.Patients in the control group were treated with oral olanzapine,and patients in the observation group were treated with olanzapine combined with repetitive transcranial magnetic stimulation.The positive and negative syndrome scale(PANSS) score and MATRICS consensus cognitive battery(MCCB) score before and after treatment were compared between the two groups.Results Before treatment,there was no statistically significant difference in the PANSS positive symptom score,general psychopathological score,negative symptom score and total score between the two groups(P>0.05).After treatment,the positive symptom score,general psychopathological score,negative symptom score and total score of the two groups were obviously lower than those of this group before treatment;the general psychopathological score(30.12±2.78) points,negative symptom score(16.24±1.48) points and total score(63.82±10.15) points of the observation group were obviously lower than(35.29±4.15),(19.13±2.75) and(71.55±10.53) points of the control group;all the differences were statistically significant(P<0.05).After treatment,there was no statistically significant difference in positive symptom score between the two groups(P>0.05).Before treatment,there was no statistically significant difference in total score of processing speed,trail making test A,animal naming score and symbol coding score between the two groups(P>0.05).After treatment,the total score of processing speed,trail making test A,animal naming score and symbol coding score of the two groups were obviously higher than those of this group before treatment;the total score of processing speed(32.27±2.34) points,trail making test A(34.57±4.38) points,animal naming score(38.47±4.27) points and symbol coding score(26.45±5.13) points of the observation group were obviously higher than(30.59±2.28),(31.26±5.79),(34.29±5.24) and(21.39±5.34) points of the control group;all the differences were statistically significant(P<0.05).Conclusion Olanzapine in combination with repetitive transcranial magnetic stimulation can obviously relieve the symptoms of patients with schizophrenia,and improve their cognitive function.
