阴道分娩次数≤3次女性盆腔脏器脱垂量化分度的影响因素

Influencing factors of pelvic organ prolapse quantitation in women with vaginal delivery times≤3

目的探讨影响阴道分娩次数≤3次的女性盆腔脏器脱垂(POP)量化分度(POP-Q)的因素。方法选取2018年1月1日-2021年1月1日于昆明医科大学第一附属医院因POP住院的女性患者297例,所有患者经阴道分娩次数≤3次,其中POP-Q(Ⅰ~Ⅱ度)者178例,POP-Q(Ⅲ~Ⅳ度)者119例。整理患者的基本病例资料,进行单因素卡方检验及多因素logistic回归分析。结果①单因素卡方检验显示,在阴道分娩次数≤3次的女性中POP-Q(Ⅰ~Ⅱ度)组与POP-Q(Ⅲ~Ⅳ度)组在年龄段、绝经、高血压、高血糖、引起腹压增加的疾病中的差异具有统计学意义(P<0.05),而阴道分娩次数差异无统计学意义。②多因素logistic回归分析显示在阴道分娩次数≤3次的女性中,年龄段和绝经是影响POP-Q(Ⅲ~Ⅳ度)发生的重要危险因素(P<0.05);而高血压、高血糖、引起腹压增加的疾病于多因素分析中无统计学意义。结论对于阴道分娩次数≤3次的女性,阴道分娩次数的增加不会引起盆腔脏器脱垂的加重,年龄的增加和绝经是影响盆腔脏器脱垂加重的主要原因。

Objective To explore the factors affecting pelvic organ prolapse quantitation in women with number of vaginal deliveries≤3 times.Methods A total of 297 women hospitalized for pelvic organ prolapse(POP)in our hospital during Jan.1,2018 and Jan.1,2021 were selected.The number of vaginal deliveries of all women was≤3 times,including 178 patients with POP-Q(gradeⅠ~Ⅱ)and 119 with POP-Q(gradeⅢ-Ⅳ).The clinical data were analyzed with univariate chi square test and multivariate logistic regression.Results①Univariate chi square test showed significant differences between POP-Q(gradeⅠ~Ⅱ)group and POP-Q(gradeⅢ-Ⅳ)group in age,menopause,hypertension,hyperglycemia and diseases increasing abdominal pressure(P<0.05),but there was no significant difference in the number of vaginal delivery.②Multivariate logistic regression analysis showed that age and menopause were important risk factors for the occurrence of POP-Q(gradeⅢ-Ⅳ)(P<0.05).However,hypertension,hyperglycemia and diseases increasing abdominal pressure had no statistical significance.Conclusion For women with vaginal delivery≤3 times,not the increase of vaginal delivery times but older age and menopause will cause the aggravation of pelvic organ prolapse.
