摘要
目的:探究经鼻持续气道正压通气(NCPAP)在重症新生儿肺炎治疗中的应用价值。方法:选取2019年1月-2020年1月中山大学附属第五医院收治的67例重症新生儿肺炎患儿,随机分为两组。观察组34例,以NCPAP为重点试验方案;对照组33例,以常规疗法为对照试验方案,比较两组患儿的呼吸频率、平均动脉压、心率、血气指标及并发症发生情况。结果:治疗后,观察组的呼吸频率、平均动脉压和心率均低于对照组,差异有统计学意义(P<0.05)。治疗后,观察组的pH值、经皮动脉血氧饱和度及氧合指数均高于对照组,差异有统计学意义(P<0.05)。治疗后,观察组的并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论:在重症新生儿肺炎的治疗过程中,采取NCPAP,可有效改善患儿的心率呼吸和血气水平,治疗安全有效,并发症少。
Objective: To explore the application value of nasal continuous positive airway pressure(NCPAP) in the treatment ofsevere neonatal pneumonia. Methods: A total of 67 children with severe neonatal pneumonia treated in the Fifth Affiliated Hospitalof Sun Yat-sen University from January 2019 to January 2020 were selected and randomly divided into two groups. Theobservation group(34 cases) was given NCPAP as the key test scheme and the control group(33 cases) was given conventionaltherapy as the control test scheme. The respiratory rate, mean arterial pressure, heart rate, blood gas indexes and incidence ofcomplications were compared between the two groups. Results: After treatment, the respiratory rate, mean arterial pressure andheart rate in the observation group were lower than those in the control group, and the difference was statistically significant(P<0.05). After treatment, the pH value, percutaneous arterial oxygen saturation and oxygenation index in the observation group werehigher than those in the control group, and the difference was statistically significant(P<0.05). After treatment, the incidence ofcomplications in the observation group was lower than that in the control group, and the difference was statistically significant(P<0.05). Conclusion: In the treatment of severe neonatal pneumonia, NCPAP can effectively improve the heart rate, respiration andblood gas level of children. The treatment is safe and effective with less complication.
