摘要
目的:探讨研究慢性肺源性心脏病患者急性加重期N端脑钠肽前体(NT-proBNP)、血清白蛋白、二氧化碳分压(PaCO_(2))及肺动脉收缩压(PASP)、三尖瓣反流程度(TR)与病情严重程度的相关性,探讨急性期与缓解期上述参数的变化情况。方法:选择2017年6月-2020年8月东阿县人民医院慢性肺源性心脏病急性加重期患者56例作为研究对象。患者急性加重期入院当天的NT-proBNP、血气分析、血清白蛋白及PASP、左室射血分数(LVEF)、TR程度与患者治疗后病情缓解期复查的上述各参数进行比较。结果:慢性肺源性心脏病急性加重患者入院时估测的PASP与LVEF、TR及NT-proBNP与病情存在正相关;急性发作期PASP、TR、NT-proBNP及PaCO_(2)水平高于缓解期,急性发作期血清白蛋白低于缓解期,差异具有统计学意义(P<0.05)。结论:PASP水平、TR程度、NT-proBNP水平、白蛋白及PaCO_(2)水平能在一定程度上反映慢性肺源性心脏病急性加重患者的病情严重程度,并可作为缓解期的评估观察指标。
Objective:To investigate the correlation of N-terminal pro-brain natriuretic peptide(NT-proBNP),serum albumin,partial pressure of carbon dioxide(PaCO_(2)),pulmonary artery systolic pressure(PASP),and degree of tricuspid regurgitation(TR)with the severity of the disease,to explore the changes of the above parameters in the acute phase and remission phase.Methods:Fifty-six patients with acute exacerbation of chronic pulmonary heart disease in Dong’e County People’s Hospital from June 2017to August 2020 were selected as the research subjects.The NT-proBNP,PaCO_(2),serum albumin,PASP,LVEF,and the degree ofTR on the day of admission in the acute exacerbation phase were compared with the above parameters in the remission phase aftertreatment.Results:PASP,LVEF,TR and NT-proBNP estimated at admission were positively correlated with the disease inpatients with acute exacerbation of chronic pulmonary heart disease.The levels of PASP,TR,NT-proBNP and PaCO_(2)in the acuteattack period were higher than those in the remission period,and the serum albumin level in the acute attack period was lowerthan that in the remission period,and the differences were statistically significant(P<0.05).Conclusion:The level of PASP,TR,NT-proBNP,albumin and PACO_(2)can reflect the severity of chronic pulmonary heart disease in patients with acute exacerbationto some extent,and can be used as the index of evaluation and observation in remission period.
